The Genesis Center exists to be a place of hope, transformation and empowerment for individuals on their journey to recovery from substance use disorders and mental health concerns. Our partnership with faith-based sober living facilities in the community gives individuals access to services that they require on their journey to sobriety. The Genesis Centers provides a full spectrum of outpatient services that includes Partial Hospitalization Programming (PHP), Intensive Outpatient Programming (IOP), Case Management, Peer Support, Individual Counseling, and Aftercare Services. Our community partnership provides clients an opportunity for stability that comes from a longer program, approximately six months in length. This affords them an increased opportunity for further development of sobriety as a state of mind and spirit that contributes to the betterment of our clients, their loved ones, and the community at large.

Job Description:

Under the direct Supervision of the Clinical Director and Site Manager, The Case Manager will provide and/or coordinate a caseload of adults diagnosed with Substance-Use Disorders. The Case Manager will be responsible for range of (trauma focused and evidence based) interventions and assessments for individuals receiving outpatient Substance Use treatment and extended follow up services providing key activities such as: assessment of client needs, linkage and referrals to community resources, development and implementation of individualized service plans. The Case Manager will ensure continuity of care for each individual and remove barriers to support recovery as they transition through multiple levels of care.

Essential Position Duties and Responsibilities:

· Accepts newly assigned cases in which a licensed professional has competed a Diagnostic Assessment.

· Completes a Case management/Needs Assessment within 48 hours of new client admission, at appropriate intervals or change in Level of Care.

· Provide ongoing supportive and or/case management functions in accordance with the problems, needs, strengths, and strategies identified within the service plan in order to achieve stated goals and objectives

· Coordinates Psychiatric and MAT appointments with TGC Nurse Practitioner or community provider, which includes making arrangements for or providing transpiration.

· Connect clients to various government benefit programs including SNAP, Medicaid, Lifeline, housing, etc. as needed and appropriate to support client in the treatment program

· Maintains communication and coordination with health care providers, behavioral health providers, medication assisted treatment programs, and legal systems to remove barriers to recovery

· Provide case management sessions, with each client on caseload at a minimum of one time per month

· Document all service contacts within 48 hours of contact including face to face encounters, as well as collateral and networking contacts

· Responsible for maintaining minimum weekly productivity

· Demonstrate appropriate and complete documentation of treatment services in compliance with agency and OHMHAS and Medicaid standards

· Participate in interdisciplinary team meetings, interagency planning and service coordination as directed

- Participates in in-service training, clinical supervision, case staffing, consultations, utilization review meetings, and other job-related meetings.

· Maintains collaborative relationships in the community as a representative of The Genesis Center to form collaborative community partnerships and intervention strategies

· Performs other duties as assigned by the supervisor, which are consistent with the policies and procedures of The Genesis Center

Abilities:

· Strong knowledge of Substance-Use Disorders and DSM 5 disorders of adults.

· Knowledge of utilizing assessments, treatment planning, coordination of benefits, and coordination for referrals between community partners.

· Excellent written and verbal communication skills.

· Proficient use of computers, programs, and EHR (Electronic Health Records)

· Documentation and maintenance of client records while maintaining privacy, appropriate boundaries, and confidentiality

· Experience with counseling, social work, and/or providing therapeutic supports to individuals and groups

Education and Licensure:

· A Bachelors Degree earned from an accredited college or university in Human Services or a related field

· Ohio Licensure by the State of Ohio Counselor & Social Work Board is preferred CDCA, LSW/LISW, LPC/LPCC or LCDC II or Higher

· Must have and maintain a valid Ohio Driver’s License, personal vehicle, appropriate driving record and appropriate insurance at all times

Work Experience:

· Prefer two years of documented experience in a related mental health services field with experience in the substance abuse services.

Physical Requirements:

· Ability to carry boxes weighing 25 pounds
