Transitional Coordinator

Description

The Transitional Coordinator will work in collaboration with the youth, family/caregiver, community resources, and members of the treatment team to identify and develop transition plans for discharge.  In efforts to encourage long term success, the Transitional Coordinator will work with the team to ensure that residential treatment strategies and approaches encourage the development of skills that are generalizable to the home community and family setting.

Job Responsibilities 
1.    In collaboration with clinical team members, work with family, relative, and community supports to identify available, long term, home community resources.
2.    Engage family/caregiver in the residential treatment process to ensure that treatment approaches are generalizable to the home setting.
3.    Provide supportive outreach by conducting visits with family/caregiver in the home setting and becoming familiar with community resources and supports.    
4.    Establish and maintain relationships with treatment providers in various communities to ensure continuity of care when youth transition out of the RTF and into the care of other providers.
5.    Empower families and ensure they are educated on the services available in their communities and can access those services when needed.
6.    Attend treatment team meetings and advocate for and ensure that treatment strategies are transferable to home setting. 
7.    Ensure that youth and families are linked to post discharge resources in their home community by participating in meetings/appointments such as Committee for Special Education (CSE), therapeutic and supportive services, and preplacement visits, during placement at RTF and post discharge. The length of Transitional Coordinator involvement with the youth and family post discharge is determined by the treatment team an assessment of individualized family needs.
8.      Act as liaison between criminal and family court systems and provide recommendations on behalf of the treatment team.
9.      Provide transportation to and from therapy and treatment team meetings.
10.    Manage a "wrap around" fund budget within agency and OMH guidelines.
11.    Maintain appropriate documentation of the provision of services in a timely and accurate manner.
12.    Successful completion of all training as defined by agency/OMH policy.
13.    Active participation in program and agency-wide performance improvement activities.
14.    Other associated tasks as directed by supervisor.

Qualifications

1.  Bachelor's degree in the human services field or a NYS Teaching Certificate and two years' experience in a behavioral health setting, preferably working with children.

2. Valid NYS Driver's License and proof of safe driving history.
