Community Stabilization Counselor-
Duties and Responsibilities:
· Assists with crisis intervention;
· Provides direct, one-on-one crisis interventions with and/or on behalf of the client to develop interpersonal, independent living, functional skills and community relational skills
· Responds to crisis situations within 24 hours
· Link clients with external programs or services, such as health services, substance use disorder services, recreational activities, childcare services, financial resources, employment resources, childcare, AA/NA groups, transportation resources, and others
· Complete case management tasks, coordinating and collaborating with other service providers, and linking clients with external programs or services; case managers, probation/parole officers and care coordinators.
· Provides therapeutic mentoring and individual supportive services;
· Utilizes effective listening skills to establish rapport, provide emotional support and encouragement, and engage family members in the service process;
· Models effective life skills with client and family and provides various skill building activities;
· Involves client in positive recreational, entertainment and educational activities;
· Maintains communication with probation officers and other authorities to monitor the client’s behavior and needs;
· Assists clients in practicing effective communication, problem solving, and anger management techniques to support daily living;
· Provides practical assistance and advice on daily living tasks such as budget management, transportation, employment, time management, and other tasks to promote independence;
· Observes client’s interactions and behavior and shares observations with Licensed Professionals to better enable Licensed Professionals to identify treatment needs;
· Participates in wraparound team meetings in ongoing effort to enlist existing supports and identify problem areas and needs, strategies, and service objectives;
· Participates in Client Specific Team, Family Assessment and Planning Team, and other interdisciplinary, diagnostic or planning meetings;
· Conducts discharge planning to ensure client is transitioned to the appropriate services for continued treatment
· Documents case activities for both service and billing purposes;
· Updates team leader on workload, case activity, service barriers, and procedural problems in staff and other meetings; and
· Attends weekly, bi-weekly, or monthly trainings and meetings
Minimum Requirements:
Education and Experience:
Must be a board Registered Qualified Mental Health Professional (QMHP), a Licensed Mental Health Provider (LPC, LCSW), Resident in Counseling or Supervisee in Social Work
CPR/1st Aid
TOVA
General knowledge of:
· client and adolescent development;
· family dysfunction factors and contributors;
· behavioral management techniques;
· local community resources and services;

Job Types: Full-time, Part-time
Pay: $40.00 - $60.00 per hour
