The Plus Group is looking for a mission-driven Spanish-speaking Nurse Practitioner (FNP) to join a dynamic, multidisciplinary, Complex Care team to advance their goal of improving health outcomes for individuals with complex medical, behavioral health, and psychosocial needs.
Responsibilities
- Manage a panel of complex patients by delivering specialized primary care strategies including street medicine.
- Lead MCC’s Enhanced Care Clinic efforts to stabilize high-risk patients using alternative care structures.
- The Nurse Practitioner Implements comprehensive patient care plan in partnership with patients.
- The Nurse Practitioner will triage and provide urgent care/walk-in visits.
- Provide health coaching and education to increase self-efficacy among target population.
- Lead/maintain key community partnerships (homeless shelters, homeless outreach agencies).
- Work with patients/case managers to address social determinants of health (SDOH).
- Track referrals and provide follow-up to ensure service delivery.
- The Nurse Practioner assists patients to coordinate all necessary care.
- Conduct home/community visits as needed;
- Consult/coordinate with interdisciplinary care team, including behavioral health, care navigation MAs, nursing, medical providers, and external partners.
- Document all Patient interactions and services using MCC’s electronic health record AND secondary database.
- Attend internal and external meetings (i.e. case conferencing, administrative).
- Attending trainings such as health coaching, engagement techniques, etc.
- Timely and accurate reporting of data as requested to MCC CM programs.
- May travel between clinics or meet Patients in the community as needed.
- Provide frank and considered opinions regarding case management services and quality improvement measures.
- Provide services to satellite clinics as needed; may be asked to work variable schedule during extended-hour clinics (e.g., 10am-7pm on one or more days).
- Other duties may be assigned.
Qualifications
Education and Experience:
- Family Nurse Practitioner (FNP).
- 1-2 years’ experience (and passion for) providing support to individuals with chronic and complex BH needs, complex medical conditions and/or substance-using individuals required.
- Knowledge of state regulations and legal/ethical standards related to patient rights, and client/patient confidentiality required.
- Training in motivational interviewing highly desired/or willingness to learn.
Required Skills and Abilities:
- Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence in English.
- Ability to communicate with patients and partners in Spanish: effectively present information and respond to questions from clients, customers, and the general public in Spanish. Candidate must be able to conduct clinical assessment interviews and case management interventions with patients in Spanish.
Computer Skills:
- To perform this job successfully, an individual must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.
Certificates, Licenses, Registrations:
- CPR Training Current.
- Valid Driver’s License.
Physical Requirements:
- Prolonged standing and walking
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
- Some local travel among clinics and care partners.
All employees are required to be fully vaccinated for COVID-19, including current booster as a condition of employment, subject to limited exemptions. New employees are required to provide proof of being fully vaccinated for COVID-19 and boosted before the first day of employment. If you completed your primary series and early boosters, but have not received the most recent booster, you will be required to get the most recent booster and mask until completed.
