Resident Care Director β Case Manager
REPORTS TO: Executive Director
DEPARTMENT: Resident Care
FLSA STATUS: Exempt

LOCATION: Maiden Park, Rochester NY
POSITION SUMMARY:
The Resident Care Director is responsible for the overall implantation, delivery, and
coordination of all resident care services within the community. This includes maintaining
excellent resident care, high occupancy and accountability for meeting community financial
goals, supervision and oversight of all care staff and compliance with both state regulations
and Willow Ridge Senior Living policies and procedures.
QUALIFICATIONS REQUIRED:
- Bachelor's degree with major course work in human services AND one year of full
time work experience in a related field OR
- Associates degree with major course work in human services OR three years of full
time work experience in a related field.
- A registered nurse, licensed practical nurse or licensed vocational nurse licensed to
practice as required by state regulations preferred.
- Approval by Department of health or other regulatory agencies per state
requirements required.
- Case Management experience preferred.
- Experience and Knowledge in all aspects of senior living, including assisted living,
Memory care, and Enhanced Assisted Living preferred.
- Knowledge of and/or ability to quickly master an understanding of related regulatory
and compliance requirements required.
- Experience in recruiting, training, and managing people in a healthcare field
preferred.
- Working knowledge of computers including Excel, Word, and MS Office.
- Compassion for and desire to work with the elderly
- First Aid Certified.
RESPONSIBILITIES:
- Supervise, develop, and schedule all resident care staff according to WRSL policies,
procedures, and standards.
- Establish and maintain a medication system which adheres to state, federal and
company laws, regulations, and guidelines.
- Perform evaluations and reevaluations to determine appropriate residency, resident
needs and then assist in establishing appropriate service plans.
- Orient and assist all residents to adjusting to life in the community.
- Operate the Resident Care Department within budget and according to WRSL policies,
procedures, and standards.
- Responsible for ordering and maintaining adequate Resident care department
supplies.
- Complete quality management reviews to ascertain if the department is meeting state
regulations and WRSL policies and follow up as needed.
- Act as a liaison for all aspects related to resident care within the community.
- Responsible for appropriate staffing levels needed to meet the needs of the facility
and state guidelines, to include filling in for staff during unexpected shortages.
- Administer medications as allowed per state specific and WRSL guidelines when
required.
- Provide resident care as allowed per state specific and WRSL guidelines when required.
- Provide training and orientation to new Resident care department staff members and
ongoing training to current department staff members.
- Responsible for all documentation completed by resident care department staff,
including 24-hour logs, care task charting, toileting schedules, and all other
documentation implemented for resident care.
- Communicate to residents, families, department heads, resident care department
staff, and Executive Director as needed.
- Work with the care department and others to promote and protect resident rights
and the psychological well-being of each resident.
- Support the move in process with a positive attitude and image of WRSL.
- Maintain active community and professional ties with clinical and non-clinical contacts.
- Responsible for tracking attendance of all care department staff and effectively
carrying out disciplinary action where appropriate and per WRSL policy.
- Communicate and enforce policies and procedures and effectively recommend
corrective and disciplinary action to enforce such policies.
- Create and maintain accurate care plans that meet the needs of the residents.
- Hold care meetings with residents and their families to assist with communication.
- Proactively tack, trend and report both resident and employee incidents and accidents
to prevent recurrence.
- Ensure or provide therapeutic interventions to assist residents in coping with their
transition and adjustment to facility, including their social, emotional, and
psychological needs.
- Orient and assist each resident to adjust to life in the community.
- Assist each resident to maintain family and community ties and to develop new ones.
- Encourage resident participation in community and community activities.
- Assist residents in need of alternative living arrangements to make and execute a
sound discharge or transfer plan as needed per state regulations and WRSL policies and
procedures.
- Assist Executive Director with timely submission of state required event reports.
- Implement and maintain an accurate medical record for each resident.
- Interact with all resident's physicians, pharmacists, and medical personnel as needed
and required per regulations and WRSL policies.
- Monitor for resident changes of condition to assists with move-out management,
preventing premature &
unnecessary resident transfers and discharges.
- Coordinate all resident care through third-party providers such as therapy, home
healthcare & hospice.
- Supervise and manage the medication administration and/or assistance with self
administration of medication programs as required by state law and WRSL policies and
procedures.
- Supervise and manage the storage, distribution, and discontinuation of resident's
medication.
- Develop and maintain positive relationships with staff at all levels of the organization.
- Ensure that residents and families receive the highest quality of service in a caring and
compassionate atmosphere which recognizes the individuals' needs and rights.
- Maintain an up-to-date Health Commerce account.
- Acquire, and maintain, an understanding of, and comply with, all Willow Ridge safety
practices, policies and procedures related to your job.
- Participate and present in annual safety training and in services.
- Monitor your staff's compliance with safety policies and procedures, implement
corrective actions when indicated.
- Report any staff injury or accident in a timely fashion for your department.
- Actively participate in employee injury/accident investigations in your department.
- Rotate as a member of the Safety Committee.
- Maintains a valid driver's license for car travel as needed.
- Performs all other duties as needed or requested by supervisor.
LEADERSHIP:

- Coach and evaluate staff performance as necessary to maintain high standards of
quality care, financial accountability, and business growth.
- Evaluate the performance and effectively recommend changes to terms and
conditions of employment for all resident care staff.
- Interview and hire all community care department staff.
- Uses critical thinking and sound judgement on decisions that impacts residents,
associates, and general operation of the community.
- Ability to assess and understand customers' expectations, needs and circumstances.
- Ability to speak intelligently about Willow Ridge and of assisted living programs.
- Assist the Executive Director with prompt follow-up and resolution of customer
service complaints and inquiries.
- Support community staff in infection control procedures including the COVID-19 and
blood borne pathogen exposure plan.
- Assist the community with sales
marketing outreach activities including routine
visits to physician's, discharge planners and other referral sources.
- Assist and participate in the community's orientation program and ongoing lnservice
program.
- Participate in the developing, reviewing, and implementing of policies concerning
resident care and quality of life.
- Maintain a high level of confidentiality regarding residents, staff, and the community.
- Rotating a manager on duty schedule with other directors.
- Rotates an on-call schedule with the Resident Care Coordinator to ensure support 24/7
for resident care department.
- Maintain flexible work schedule to be available to work evenings, nights, weekends,
and holidays as needed.
- Availability by phone or page 24 hours a day, 7 days a week for emergencies.
- Follow Willow Ridge Senior Living policies and procedures.
PHYSICAL/SENSORY REQUIRMENTS:
- Ability to exert 20-50 pounds of force occasionally and/or 10-25 pounds of force
frequently and/or up to 10 pounds of force constantly to lift, carry, push, pull, or
otherwise move objects.
- Additional physical demands include standing, walking, sitting, stooping, kneeling,
crouching, crawling, bending, twisting and generally all movement associated with
resident care.
- Travel frequently by car.
- Must be in full compliance with all regulatory required vaccinations (including COVID-
19, MMR, etc).
OSHA CLASSIFICATIONS:
- Potential for exposure to bloodborne pathogen.
