These services enable individuals and families to build and sustain productive, meaningful, and happy lives.
Purpose :
To work in collaboration with the client, family and professional team to establish and work towards meeting goals and objectives as outlined in the Individualized Treatment Plan. As well as, to provide collaboration that assesses, plans, links, coordinates, and monitors options and services that address a member's needs.
Qualifications :
- 18 years or older
- Bachelor's level degree in a health or human services field
- Detail- Oriented / Organizational Skills
- Professional and friendly with public
- Enjoy Therapeutic Environment
- Current Enhanced background check
