What are the responsibilities and job description for the Direct Support Professional - Extended Support position at UCP Seguin of Greater Chicago?
PRIMARY FUNCTION: Provide supportive assistance, personal care & training to consumers in their daily living, social, recreational, day services and community activities, in accordance with the goals & requirements prescribed in each consumers I.S.P. and in a manner that facilitates cross-team collaboration, cooperation and service integration across work unit, department and organizational boundaries.
WORK SCHEDULE: This position is scheduled as "6 days on, 7 days off", for a total 144 continuous hours on site. There will be at least 5 hours of uninterrupted sleep time built into every 24 consecutive hours. The Extended Support Specialist is paid an hourly rate with guaranteed scheduled overtime. Over time is paid after 80 straight hours have been worked in each work week. The work week is defined as 12:00 am on Sunday- 11:59 pm on Saturday. The Extended Support Specialist schedule typically begins on a Wednesday at 9 am and works until the following Tuesday at 9am or Thursday at 9am and works until the following Wednesday at 9am.
QUALIFICATIONS:
EDUCATION: High School diploma or GED
AGE: Minimum of 21 years of age
EXPERIENCE: 1 year of successful work experience with persons with developmental disabilities.
LICENSE/CERTIFICATIONS: Valid drivers license required and must be approved by insurance company as a driver for the agency.
Certificate of Proof of Insurance and maintenance of such at minimum amount of coverage required by law. Possess or acquire within first ninety days of employment self-administration of medication training. Maintain certification through retraining and annual recertification, as required.
Possess or acquire within new hire training cycle First Aid and C,P.R. certification, updating as required by licensing agency.
Completion of Department of Human Services mandated training curriculum for DSPs in Illinois within agency designated timeline or within 120 days (whichever is less)
Certification in Safety Care behavior intervention training within the first month of hire
WORKING CONDITIONS:
PHYSICAL DEMANDS/ACTIVITIES: Render physical assistance frequently to persons in areas of personal care and activities of daily living and implement appropriate physical interventions during behavior incidents frequently and especially during such times when providing sole supervision, to ensure their personal safety, physical needs and well being which require the following capabilities.
