Responsibilities:
- Conduct comprehensive assessments of patients' psychosocial needs
- Collaborate with healthcare professionals to develop individualized care plans
- Provide counseling and psychotherapy services to individuals and families
- Assist patients in accessing community resources and support services
- Advocate for patients' rights and ensure their needs are met
- Maintain accurate and up-to-date medical documentation
- Collaborate with interdisciplinary teams to coordinate patient care
- Provide education and support to patients and their families regarding medical conditions and treatment options
- Conduct individual and group counseling sessions as needed
Qualifications:
- Master's degree in Social Work or related field
- Valid state licensure as a Social Worker
- Experience working with senior care
- Knowledge of medical documentation standards and ICD-10 coding
- Proficiency in providing psychotherapy and counseling services
- Strong communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team setting
- Knowledge of child and family counseling techniques is desirable
- Reliable Transportation. Valid Auto Liability Insurance
This is a PRN contract position. Rate of pay and mileage reimbursement is negotiable upon discussion with the Administrator.
