The primary purpose of this position is to provide case management, develop support services and make appropriate community referrals to address individual needs of the family to ensure stabilization of housing and promote independent living. Provide intake, data collection and verification, eligibility determination, data entry, information and referral, information compilation, maintain files and other information for UCAN programs, i.e.: homeless assistance, rent/mortgage assistance, utility supports, financial independence, access to health care and tenant education.
Program Related Duties Include:
- Interviewing applicants to ensure continued eligibility for services following program and supervisor guidelines and allow accessibility to necessary supportive services that maintain housing stabilization. Set action plans that are supportive to family goals and ensure participants are actively engaged in achieving outcomes selected. Provide actionable steps to ensure outcomes are documented and when deficient work with household to explore options that lead to actionable steps that contribute to sustainability. Explain program, gather and verify information given and determine eligibility for services following guidelines of programs. Assist applicants in completion of applications. Process paperwork correctly and in a timely manner.
- Assist the family to assess their needs and develop a family plan of both short-term and long-term goals and work plan to meet those needs. Utilize UCAN programs, such as homeless assistance, rent/mortgage assistance, utility supports, financial independence, access to health care and tenant education.
- Evaluate each family’s progress point of entry toward self-sufficiency and stabilization. Work with participants to help them develop the skills needed to assume responsibility for choices and progress. Assist participants in completion of applications for services with internal and external partners.
- Provide assistance in acquiring safe, affordable and stable housing. Advocate and provide suggested referrals as applicable and work with Housing and Homeless Eligibility staff and Homeless Outreach Coordinators to ensure stabilization of sustainable housing.
- Provide participant training on money management, tenant rights and responsibilities, property care, maintenance, and to coordinate other educational services that support household members and allow access to additional supports available in the community. Ensure linkages are created for household to participate in and create plan in alignment with household for post-service maintenance.
- Remain aware of community resources; establish and maintain rapport with community partners and vendors. Provide housing information, community resource identification and referral, networking, linkages and advocacy to other community resources. Work with area landlords to establish partnerships.
- Provide follow-up services and a supportive network to encourage family stabilization and self-reliance. Provide wraparound service as directed, for immediate intervention when necessary for the health and well-being of the household and create referrals for additional supports to reduce crisis needs.
- Assist and Participate in program related events and projects such as warming and cooling center operations, Point in Time events, Partnering events, community outreach and support etc.

- Drive a company vehicle in the performance of duties. Driving will be required for travel to client homes to perform home visits, and to perform other essential functions as needed.
- Collect data needed for completion of program reports. Enter data into the HMIS system. Maintain accurate files assuring all information is entered into the programs data base in a timely fashion. Provide information and reports to Supervisor as scheduled or requested.
