Key Responsibilities:
- Develop & Review Care Plans: Collaborate with physicians, patients, and families to develop and periodically review care plans that meet the patient's needs.
- Coordinate Care: Ensure smooth communication with the interdisciplinary team and keep everyone informed about the patient’s care, including timely discharge information.
- Ongoing Patient Assessment: Regularly assess the patient's clinical, functional, and emotional status to monitor progress and identify any issues. Communicate changes to the physician and the care team.
- Education & Support: Teach patients and caregivers self-care techniques and provide health counseling, including medication, diet, and treatment instructions.
- Documentation: Document evaluations, treatment plans, goals, and patient progress in a clear and accurate manner.
Qualifications:
- Licensure: Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- Education: Graduate of an accredited nursing school.
- Experience: Medical/Surgical nursing and/or home health care experience is required.
- Skills: Strong communication, clinical judgment, and organizational skills to manage a diverse caseload.
