What are the responsibilities and job description for the Navigator position at University Health - San Antonio?
POSITION SUMMARY/RESPONSIBILITIES
Assists Program Coordinator in the implementation of the Connecting Kids to Coverage Program for University Health. This position will assist and guide patients through the completion and follow-up of applications for The Centers for Medicare and Medicaid Services (CMS) Connecting Kids to Coverage grant. This position will serve as a liaison between internal and external customers, including the health system, the community, and Bexar County agencies, in order to increase the number of community events, referrals and application submission support to clients. Additional activities will include preparing and conducting educational presentations to educate the community at large about the Connecting Kids to Coverage grant.
EDUCATION/EXPERIENCE
A Bachelor’s degree in social sciences, health education or similar field or Community Health Worker certification is preferred. A minimum of two (2) years or equivalent experience in the area of community health, education, public health, or case management is preferred. Experience providing educational presentations to both large and small groups as well as public speaking demonstrating effective communication skills is required. Previous experience navigating patients preferred. English/Spanish fluency is strongly desired.
