Job Summary: We are looking for an experienced Home Health Nurse to join our Beaverton office. The ideal candidate will have extensive knowledge of home healthcare operations, Medicare certification processes, and state-specific regulatory requirements. This role is crucial for ensuring our Beaverton office meets all necessary standards to obtain Medicare certification and provide high-quality care to our patients.
Key Responsibilities:
Assist the Beaverton office through the Medicare certification process, ensuring compliance with all state and federal regulations.
Provide high-quality patient care by performing comprehensive assessments, developing individualized care plans, and delivering skilled nursing services.
Educate patients and their families on disease processes, treatment plans, and self-care techniques to ensure optimal health outcomes.
Coordinate with other healthcare professionals to develop and implement comprehensive patient care plans.
Document all patient interactions, care plans, and outcomes in accordance with regulatory and agency standards.
Monitor and report changes in patient conditions to the healthcare team promptly.
Participate in quality improvement initiatives to enhance patient care and ensure compliance with Medicare standards.
Stay up-to-date with changes in home healthcare regulations and ensure compliance with current standards.
Collaborate with the clinical team to ensure the Beaverton office meets the overall strategic goals of Vancouver Home Health Care Agency.

Qualifications:
Bachelor's degree in Nursing (BSN) or related field.
Current, unencumbered RN license in the state of Oregon.
