Job Details
  • Management/Supervision: Meets with Health Promotion staff to provide direction to their respective duties and responsibilities. Supports and motivates staff in identifying and completing professional development goals.
  • Program Management: Plans, implements, and organizes public health programs and activities, including environmental health needs. With the assistance of the Director of Outreach manages programming. Leads implementation and evaluation of community-based prevention and cessation programs that focus on tobacco product usage, diabetes prevention, safe sleep, employee wellness and other identified programming. Coordinates the delivery of intervention(s) consistent with the implementation plan. Delivers health education and promotion interventions. Monitors implementation and assesses progress in achieving objectives.
  • Needs and Assets Assessments: Plans assessments, defining the purpose and scope of the assessment. Identifies priority populations, existing and available resources, policies, programs, practices, and interventions. Obtains and identifies primary data, secondary data, and other evidence-informed sources and determines its validity and reliability. Conducts community and participant surveys to ascertain health needs, develop desirable health goals, and determine availability of services. Analyzes the data to determine the health of the priority population(s) and the factors that influence health. Synthesizes assessment findings to inform the planning process.
  • Program Planning: Engages priority populations, partners, and community organizations for participation in the planning process. Defines desired outcomes. Determines health education and promotion interventions, including creating logic models and pilot testing. Develops plans and materials for implementation and evaluations. Works with partners and community members to evaluate educational materials to determine appropriateness (cultural & linguistic) for clients being served.
  • Program Evaluation: Designs process, impact, and outcome evaluations of the intervention. Completes Quality Assurance on individual programming. Designs research studies. Manages collection and evaluation of data using appropriate technology. Interprets data. Uses findings to improve programs and reports to VCHD leadership, community stakeholders and meet grant objectives.
  • Local/Regional/State Liaison: Serves a role in local coalitions, regional initiatives or groups, and state level coalition groups. Participates in coalition meetings. Coordinates relationships with partners and community organizations. Provides training to expand capacity of partner organizations. Promotes and represents programs at community events and at internal activities. Recruits health care providers/clinicians, community and faith-based organizations, and other partners to promote programs program to clients. Serves as facilitator in coordinating meetings and distribution of information. Works with community members to assist with cultural and linguistic educational materials for target population.
  • Writing/Communications/Marketing: Prepares written and digital materials on public health related issues in conjunction with VCHD leadership. Assists in developing marketing plans to promote programming efforts. Develops effective messaging for target audiences. Selects methods and technologies to deliver messages and delivers messages effectively using identified strategies. Prepares and provides presentations in the community. Develops and circulates programmatic newsletter to clients and general public.
  • Grant Writing/Reporting: Assists with writing, editing, and collecting documentation to ensure full compliance with the grant including deliverable outcomes. Assists with identifying and recommending grant expenditures. Prepares submission of State and/or Federal grants as appropriate and is responsible for managing program budget, reporting and grant requirements. Researches and prepares grant applications, prepares grant reporting documents. Creates required reports reflected of program services.
  • Employee Wellness: Incorporation of wellness and disease prevention within the workplace. Including to reach or remain at a healthy weight, tobacco cessation, stress management, improved biometric screenings and optimize their physical strength and stamina.
  • Provider Relations: Manages a community network system of existing service providers to offer essential services to residents. Assists in identifying local health resources that could be mobilized to improve individual and community outcomes.
Experience/Qualifications
  • Qualifications: Bachelor’s degree in public health, behavioral health, health education/promotion, health sciences, public administration, healthcare, substance abuse, mental health, or related field required. CHES certification and/or master’s degree in public health education or related field (public health, adult education, community health, health promotion) from an accredited college or university preferred. Three to Five (3-5) years of directly related experience. Public speaking: 2 years. Grant writing: 2 years (preferred).
  • Knowledge: Knowledge of effective health promotion education principles and methods as well as health education theory. Knowledge of health equity. Principles and practices of community outreach and engagement as well as health communication. Social media and other digital communication methods. Agency policies/procedures. Grant application process. Knowledge of and familiarity with county community resources to adequately perform job. Research methods to gather data. Knowledge of marketing and communications principles and presentation techniques for culturally diverse audiences. Knowledge of social science assessment, intervention, and consultation principles and practices. Knowledge of community organization. Knowledge of program management, planning, evaluation, needs assessment, and grants management. Knowledge of adult learning theory and implementation preferred.
  • Skills: Ability to develop and present effective health promotion marketing and educational materials in a variety of settings. Skill in organizing work activities to meet established objectives. Skill in communicating effectively in writing and orally to individuals and groups. Skill in applying interviewing and assessment techniques for culturally and socioeconomically diverse populations. Computer skills including MS Office. Data analysis skills. Program management skills. Skills in conducting literature reviews.
  • Abilities: Effective written and verbal communications skills, including public speaking, marketing, and design. Experience in grant writing preferred. Experience in facilitation and public speaking. Ability to present programs and materials in a culturally sensitive manner and to use literacy and health equity considerations in the development and delivery of education. Ability to work in collaboration with community partners. Ability to establish and maintain effective working relationships with a diverse population of community members, medical providers, businesses, schools, community-based agencies, and departmental staff. Ability to collect and maintain program data and confidential information. Leadership abilities.
  • Professional Development: Attends continuing education seminars, certifications conferences, and meetings to improve/acquire knowledge of current trends and evidence-based practices in public health. Joins and participates in nursing associations, seeks certification, returns to academia and/or presents at a regional or national conference. Is sought out to represent the public health nursing profession by community partners and stakeholders. Demonstrate professionalism in by wearing appropriate uniforms or dress clothes for the situation.

  • Training and Orientation of Staff: Responsible for training new employees on programming, documentation, providing orientations schedules, and assess the need for more training, extended orientation or release from orientation.
  • Licenses/Certifications: Certified Health Education Specialist (CHES) preferred. Certified Tobacco Treatment Specialist (CTTS) preferred and other certifications as identified.
  • Physical Requirements: Clinical and administrative work setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. Desk and workspaces are able to accommodate a wheelchair. Position uses standard medical equipment to perform assessments.
  • Travel: Driving a vehicle requiring a license. Some travel is required, and is typically local or regional, to meet with clients, conduct business, or attend or provide training.
  • Background Check: Pass a drug screen and criminal background check.

  • Flexible work hours; 40-hour work week with very occasional weekend assignments. Full time, usual hours 8:00 a.m. – 4:30 p.m., M–F, with lunch breaks. May be required to work occasional evenings with the ability to flex these hours.
