The Opportunity: We are looking for a Licensed Practical Nurse (LPN) to join our team on a full-time basis. This role requires 40 hours per week, Monday-Friday. This individual will be responsible for Population Health initiatives including patient outreach and quality metrics, as well as patient care.
Responsibilities: As a LPN at Vanguard Medical Group, you will be an essential part of our patient-centered care team. Your responsibilities will include:
- Patient Preparation
- Ensure that examination rooms are well-prepared with instruments, supplies, and equipment.
- Verify patient information, review medical history, and record vital signs.
- Greet and prepare patients for their appointments, ensuring a welcoming and professional experience.
- Support healthcare providers in patient care, including assisting with examinations, minor procedures, and treatments.
- Administer medications and injections as prescribed.
- Perform diagnostic tests such as EKGs, urinalysis, and point-of-care testing.
- Collect and prepare specimens for laboratory analysis.
- Provide patients with information about their medications, treatment plans, and lifestyle modifications.
- Answer patient questions to ensure they understand their healthcare plans.
- Maintain accurate medical records by documenting patient examinations, treatments, and test results in the electronic medical record (EMR) system.
- Safeguard patient care information, ensuring confidentiality and protecting operations.
- Monitor and document key quality metrics, such as screenings, vaccinations, and chronic disease management, to ensure compliance with population health initiatives.
- Assist with identifying and closing care gaps for patient populations by reviewing reports and following up with patients as needed.
- Collaborate with the care team to meet practice-wide quality goals and regulatory requirements.
- Conduct patient outreach via phone or electronic communication to schedule overdue preventive screenings, chronic disease follow-ups, or wellness visits.
- Provide follow-up support for patients managing chronic conditions or post-hospitalization care to ensure continuity of care.
- Engage patients in education and support to promote adherence to care plans and healthy behaviors.
Requirements:
- High school diploma or equivalent
- Active and unencumbered LPN license in the state of New Jersey
- CPR certification (Basic Life Support)
- 1 years of experience in a medical office or hospital setting preferred
- Working knowledge of Athena or an Electronic Medical Record (EMR) system preferred
- Excellent interpersonal, communication, and organizational skills
