Essential Functions
- Delivery of Patient Care
- Participates in the assessment of the patient’s physical and mental health, including the patient’s reaction to illness and treatment regimens.
- Records and reports the results of the nursing assessment.
- Participates in implementing the Plan of Care developed by the registered nurse and/or prescribed by any person authorized by State law, by performing tasks assigned or delegated by and performed under the supervision or under orders or directions of a registered nurse, licensed physician or any other person authorized by State law to provide supervision.
- Facilitates patient and caregiver independence to the extent possible.
- Participates in the teaching and counseling of patients as assigned by a registered nurse, physician or other qualified professional authorized by State law.
- Encourages and allows time for questions when meeting with the patient and family.
- Reports and records the nursing care rendered, and the patient’s response to that care.
- Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.
- Maintains open lines of communication with supervisor. Keeps supervisor apprised of any complex patient/caregiver issues.
- Applies principles of infection control. Follows guidelines for universal precautions in accordance with accepted professional practices.
- Interdisciplinary Group
- Attends and participates in the interdisciplinary group conferences [IDG] and special care conferences.
- Seeking and utilizing the appropriate resources for the referral process, as directed by the RN
- Participates in identifying specific patient/caregiver needs and making referrals to appropriate team members.
- Documentation
- Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.
- Professional and Personal Accountability
- Demonstrates competency in knowledge and skills within scope of practice.
- Ensures that VIA Health Partners professional reputation is maintained and projected.
- Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice.
- Maintains current professional licensure, CPR certification and CEU requirements.
- Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to unforeseen obstacles.
- May perform other duties as required.
Qualifications
Minimum Qualifications
• A Diploma from a Board approved nursing program designed to prepare a person for practical nurse licensure is required;
• A current, active, and unrestricted compact (multistate) license as a Licensed Practical Nurse (LPN) is required.
• The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.
• Current CPR required; CHPN preferred.
• Minimum 1-year related experience for LDHH and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
• Computer proficiency is required.
