The Villas at Sunny Acres Post-Acute/ skilled nursing facility seeks an energetic and experienced Social Worker/Social Services !
We are seeking an experienced and detail-oriented individual who will work in the Social Services department as the Social Services for our Skilled/Rehab patients. Working in a SNF is required as this is a very fast paced environment!
This person will work with the patients directly and their families. Facilitates interdisciplinary plans in collaboration with interdisciplinary team(s), and assures Utilization review reports are completed and provided to payor as needed. Serves as a liaison between patient, physician, care team, payor, and the discharge planner by coordinating, monitoring and communicating patient’s progress and cost evaluation while in the facility. Assists with coordination to the next care level.
