Role Description
This is a part-time/ per diem on-site role for a Pharmacist at Vital Care Infusion Services in Crown Point, IN. The Pharmacist will:
Be
- responsible for dispensing medications accurately
- Review and execute physician orders, checking for accuracy, appropriateness, and legality
- Make clinical assessments of patients
- Counsel patients on proper medication use
- Consult with patients and/or other healthcare practitioners on drug interactions/contraindications, dosage, drug interactions, and side effects
- Maintain full control over delivering, stocking, and labeling of medications
- Ensure regulatory compliance with state and federal pharmacy law
Qualifications
Sterile
- compounding and dispensing skills
- Patient counseling and medication therapy management experience
- Strong knowledge of USP <797> guidelines
- Excellent communication and interpersonal skills
- Attention to detail and accuracy in dispensing medications
- Ability to work in a fast-paced healthcare environment
- Current Pharmacist licensure in Indiana
- Good organization skills
- Knowledge of clinical operations and procedures
- Working knowledge of various software programs (CareTend) and pharmacy information systems
- Work cohesively with Pharmacy Manager and Pharmacy Technicians
- Flexible schedule to allow for 24-hour notice
- Home Infusion pharmacy experience is desired
***Please note that this is a Per Diem role for the purposes of providing relief coverage to the Pharmacy Manager. Hours will not be regularly scheduled and this is an “as needed” position.***
