Licensed Certified Social Worker (LCSW)

Position Overview:

Zaif Health is committed to providing culturally sensitive and trauma-informed care to immigrant and refugee communities. Our mission is to empower individuals and families to achieve emotional well-being and resilience through personalized behavioral health services and holistic support.

We are seeking a dedicated and compassionate Licensed Certified Social Worker (LCSW) to provide in-person services to children, families, and individuals in classrooms, at our agency office, and in client homes as needed. This role involves delivering therapy, psychosocial support, and case management while fostering collaboration with schools, families, and community partners.

Responsibilities:

1. Conduct Comprehensive Assessments:

  • Perform thorough assessments of clients' mental health needs, considering their social, emotional, and environmental factors.
  • Gather and analyze relevant information to develop a comprehensive culturally appropriate understanding of each client's unique challenges and strengths.

2. Develop Individualized Treatment Plans:

  • Collaborate with clients to create personalized treatment plans that address their specific mental health goals and needs.
  • Utilize evidence-based practices and interventions to design effective therapeutic strategies.

3. Provide Therapeutic Interventions:

  • Conduct individual, group, and family therapy sessions to address a range of mental health concerns.
  • Implement various therapeutic modalities, such as but not limited to cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), trauma-informed care, and solution-focused therapy, based on clients' needs.

4. Foster Empowerment and Resilience:

  • Empower clients to develop coping skills, build emotional resilience, and promote self-advocacy.
  • Encourage clients to identify and utilize their strengths to overcome challenges and achieve positive outcomes.

5. Resource and Referral Services:

  • Provide information and assistance to clients in accessing community resources, social services, and additional support systems.
  • Facilitate referrals to external agencies and organizations that can address specific client needs beyond the scope of the agency's services.

6. Monitor Client Progress:

  • Regularly assess and evaluate clients' progress in achieving treatment goals.
  • Modify treatment plans as needed to ensure clients receive the most effective care.

7. Collaborate with Multi-Disciplinary Team:

  • Work closely with other professionals, including but not limited to counselors, psychologists, case managers, and community organizations, to provide coordinated care and support.

8. Maintain Accurate Documentation:

  • Ensure timely and accurate documentation of client assessments, treatment plans, progress notes, and resource/referral information.
  • Comply with all confidentiality and legal requirements regarding client records.

9. Stay Current with Best Practices:

  • Attend professional development opportunities to enhance skills and stay updated on the latest research and best practices in the field of social work.

Knowledge, Skills, and Abilities:

  • Must demonstrate superior customer service and people skills, being able to respond to the immediate needs of patients or supervisors
  • Must have effective oral and written communication skills, being able to effectively reach diverse populations and entities
  • Demonstrate to work independently in carrying out assignments while remaining reliable and dependable
  • Ability to demonstrate basic levels of computer literacy, with a functional understanding of Microsoft Windows Operating Systems in an office setting
  • Demonstrate a working knowledge of routine office hardware and equipment (e.g., printers, copiers, fax, machines, etc.)
  • Must be familiar with entering data into a healthcare database program (EHR)
  • Must demonstrate the ability to maintain accurate records and preparing clear and accurate reports for informational, auditing, and operational use, including capacities for attention to detail and for reviewing and verifying accuracy of data
  • Demonstrate the competency and integrity to maintain the confidentiality of organizational, employee, and client records

Requirements:

  • Master's degree in Social Work from an accredited program.
  • Licensed as a Social Worker (LCSW) in the state of Washington through the Department of Health.
  • Demonstrated experience providing counseling and therapy services to diverse populations.
  • Knowledge of evidence-based therapeutic modalities and interventions.
  • Strong interpersonal and communication skills, with the ability to establish rapport and build trusting relationships with clients.
  • Empathetic, non-judgmental, and culturally competent approach to care.
  • Ability to work effectively as part of a team and collaborate with other professionals.
  • Preferred: bilingual in English and Somali
