What are the responsibilities and job description for the Behavioral Health Liaison position at Volunteer Behavioral Health Care System?
As a Behavioral Health Liaison, you will be a resource to serve as a crisis assessor, care manager, educator and community outreach specialists. Your goal will be to enhance the efficiency of behavioral health systems and maximize opportunities to identify and connect individuals with appropriate level of care. You will support individuals who are involved with services from other community partners, and upon discharge, connect them with necessary treatment to continue their path to recovery.
Job Description:
1. Establish and maintain productive relationships with community partners. Promote VBHCS and services offered, showcasing our commitment to enhancing the overall well-being of our clients.
2. Evaluate clients to determine the level of behavioral health care they require. Provide efficient linkage to appropriate resources, ensuring each individual receives the necessary assistance.
3. Conduct crisis assessments and refer clients to the appropriate level of care for behavioral health treatment. Per scheduling needs, assist in the Walk In Center (WIC), Crisis Stabilization Unit (CSU), and/or Detox when directed to alleviate staff shortages or high demand.
4. Assist with the application process for TN Safety Net and/or TennCare as needed to encourage follow through with behavioral health treatment and care.
5. Provide education and training to individuals and agencies concerning behavioral health to increase behavioral health literacy and to reduce stigma.
6. Participate in staff meetings, training, and supervisory sessions as required.
7. Make contact with clients on the hospital admit and discharge list and link them to resources.
8. Identify resources to assist individuals, agencies, families, and other social supports.
9. Advocate/ communicate patient needs to community partners.
10. Identify and facilitate access to other community supports.
11. Provide information and education on accessing services.
12. Provide customer service by educating clients and agencies on open access/crisis services.
13. Respond to clients presenting to a hospital, ER, or community agency, and complete evaluations and data entry in an efficient manner to meet standards set forth by VBHCS.
14. Collect and document data required by grant funder(s) and for billing purposes, according to quality management standards and procedures.
15. Other duties as assigned.
QUALIFICATIONS:
Preferred applicants should be engaging, enjoy working in the community and building relationships with our community partners. They should be able to successfully collaborate with primary and specialty care providers, and Volunteer Behavioral Health ensuring clients address needs as they work toward recovery. Applicants should be able to work with a variety of clients who experience mental health issues including substance use disorders.
EDUCATION & EXPERIENCE:
Must have a Bachelor’s degree in a health-related field of counseling, psychology, social work or other behavioral sciences and at least two (2) years of paid work experience in the behavioral health setting. Knowledge of computer skills to be able to search and enter data in the Electronic Record system is required.
