The candidate will actively participate in the LEAD program by working with eligible individuals, connecting them to intensive case management services and deflecting them away from incarceration, as well as training all sworn personnel in the principles of harm reduction and LEAD. Candidates must have good verbal and written communication skills and be sufficient in working on the internet and in Microsoft Word as a routine part of daily work activities. Qualifications include possessing a CADC, or a MHRT, or Case Management experience. Must have at least one year of case management experience. Must have own vehicle and valid Maine driver’s license as travel is required to assist clients, attend client meetings, training, or other events. Position Overview: The Law Enforcement Assisted Diversion (LEAD) Case Manager (CM) is responsible for co- response with law enforcement (LE) and follow up referrals from LE to calls for services primarily related to substance use disorder (SUD) and co-occurring disorders. The CM provides SUD assessment and criminogenic risks; and connects individuals to services based off assessments. The CM assists residents with employment, housing, treatment, living skills and other issues for ensuring a successful transition into the community. Other duties include but are not limited to establishing and maintaining productive relationships with clients and their families, keeping case records, group facilitation of skill and cognitively based programs, and ensuring all activities are conducted in compliance with the funding source, program, and agency policies and procedures.

Essential Job Functions:

1. Provides case management services to assist in the diversion and deflection programs and transitions from jail and upon release.

2. Work in collaboration with diversion program manager, reentry programs manager and director of community justice programs to assist in connecting individuals booked into regional jails to either post releases services if bailed, post incarceration or on pre-trial status.

3. Manage client caseload assigned according to the program contract.

4. Assess clients using Sud assessments and criminogenic risk assessments.

5. Model appropriate behavior and maintain appropriate boundaries.

6. Build positive working relationships with clients while they are at the program.

7. Work cooperatively and coordinate with the program staff on all aspects of the Aftercare case plan.

8. Facilitate support groups as designated by the program director to address risk factors of residents.

9. Provide mentoring and support for the clients participating in the program.

10. Maintain accurate, up-to-date case records, logs and reports and submit to the Program Director.

11. Develop and maintain awareness of community resources to pursue programming and resource opportunities. Provide referrals to community resources as needed.

12. Participate in and contribute to other duties and responsibilities as assigned.

13. Complete client assessments as and when required.

14. Identify significant supports needed and develop a comprehensive, non-redundant case plan that addresses criminogenic risk factors and reflects contributions from multiple service providers, and modify the plan as client circumstances, policy mandates and needs dictate.

15. Ensure provision of gender responsive client services according to the case plan, including services such as drug and alcohol counseling/referrals and workshops, drug screening and monitoring, job development, and referral to community resources, and document those activities in each client’s case file.

16. Involve the client’s family with permission and where appropriate in the case management process.

17. Instruct clients in basic financial skills, including budgeting, savings, fiscal responsibility, and mechanisms to develop and build credit.

18. Assist clients in dealing with their total health concerns and teaching about the effective utilization or established health agencies and providing appropriate referrals for release.

19. Complete comprehensive discharge plan.

20. Provide intervention services to clients in the program service area.

21. Coordinate and facilitate cognitive/behavioral and life skills groups for clients.

22. Maintains positive working relationships both internal and external.

23. Represent the agency in collaborating with other community-based agencies to expand service networks.

Education, Experience, and Qualifications:

1. Bachelor’s degree in addiction and/or social services or related field preferred.

2. CADC/LADC licensure in the state of Maine, preferred.

3. MHRT/C certification in the state of Maine, preferred.

4. Case management experience in lieu of certification.

4. A minimum of one year of relevant, transferable experience as a case manager is required; experience working with a program-defined population is preferred.

5. Four years of experience working within the relevant field of services may be considered in lieu of a bachelor’s degree preference, or any combination of experience, training and education that effectively demonstrates required knowledge, skills, and abilities.

6. Experience in individual case planning and management, resource development, and group facilitation.

7. Ability to motivate a challenging population and achieve self-sufficiency outcomes.

8. Excellent written, oral, organizational, and record-keeping skills.

9. Ability to work flexible hours including some evenings and weekends.

10. A valid state of Maine driver’s license, a safe driving record and a reliable vehicle is required.

11. Strong computer skills, proficiency in Microsoft Word, Excel, Outlook.
