What are the responsibilities and job description for the RN Case Manager (Casual/PRN) position at Warren General Hospital?
We're seeking an experienced case management RN to assist our team in a casual/per-diem capacity; hours will vary based on need.
RN Case Manager General Description:
The RN Case Manager provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integration and functions of case management, utilization review and discharge planning. Has accountability for the care, coordination and discharge planning of all patients.
RN Case Manager Essential Job Duties:
- Coordinate the integration of case management functions into the patient care, discharge and home planning processes with other hospital departments, external service organizations, agencies and healthcare facilities.
- Conduct medical record review using McKessen Interqual criteria and the Review Manager software.
- Utilize Optum Executive Health Resources appropriately for secondary physician review.
- Promote effective and efficient utilization of clinical resources.
- Conducts review for appropriate utilization of services from admission through discharge.
- Communicates with physicians at regular intervals throughout hospitalization and develops an effective working relationship. Assist physicians to maintain appropriate cost, case, and desired patient outcomes.
- Assess patient’s progress through expected hospital course.
- Refers cases where patients and/or family would benefit from counseling required to complete complex discharge plan to social worker.
- Facilitates multidisciplinary patient care conferences to review treatment goals, optimize resource utilization, and identify post-hospital needs.
- Collaborate with clinical staff in the development and execution of the plan of care, and achievement of goals.
- Refers cases for Swing Bed as appropriate and assists physicians, patients and families with transition from outpatient or acute care to Swing Bed status utilizing knowledge and understanding of the program.
- Assists in appropriate utilization of Swing Bed.
- Adhere to departmental goals, objectives, standards of performance, and policies and procedures.
- Ensure compliance with quality patient care and regulatory compliance.
RN Case Manager Work/Education Requirements:
- Previous Case Management, Utilization Review or Quality Assurance experience.
- Associate's Degree required; Bachelor’s degree preferred.
- Must be currently licensed as a registered professional nurse in Pennsylvania.
