Warsaw Meadows Care Center, an 80-bed skilled nursing facility in Warsaw, IN, is seeking an experienced MDS Assessment Coordinator (Registered Nurse) to join our team.
This is a full-time nurse manager position, with a Monday-Friday (flexible) schedule (8:00 AM - 4:30 PM), with on-call nursing rotations.
Responsibilities:
- Conduct and coordinate resident assessments per state requirements and facility policies.
- Routinely assess residents on pre-set schedules or as needed (e.g., condition changes or hospital returns).
- Observe, assess, and communicate condition changes to appropriate personnel.
- Ensure timely completion of assessments and CAAs by interdisciplinary departments such as Social Services, Activities, and Dietary, obtaining required signatures.
- Complete MDS sections, quarterlies, CAAs, and care plans in the absence of other personnel.
- Develop and update care plans based on resident needs and Resident Assessment Protocols (RAPs).
- Prepare and distribute the MDS schedule to the interdisciplinary team.
- Transmit MDS to the state weekly (or as required) and maintain accurate records, including a transmittal log.
Qualifications:
- Current Registered Nurse (RN) license in good standing in the state.
- Minimum of 1 year of experience as an MDS Assessment Coordinator in a long-term care setting.
