Job Summary
We are seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our dedicated healthcare team. The ideal candidate will provide comprehensive care to patients with serious illnesses, focusing on improving quality of life through symptom management and support. This role requires a strong clinical background and the ability to work collaboratively with patients, families, and interdisciplinary teams.
Responsibilities
- Conduct thorough assessments of patients to develop individualized care plans that address their physical, emotional, and spiritual needs.
- Collaborate with physicians and other healthcare professionals to coordinate care for patients with complex health issues, including infectious disease care.
- Educate patients and families about disease processes, treatment options, and health coaching strategies to empower them in their care decisions.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Document all patient interactions accurately in electronic health records (EHR) systems.
- Perform Face to Face visits for Hospice Medical Director
Requirements
- Master’s degree in Nursing or a related field with a focus on Palliative Care or Hospice.
- Current Nurse Practitioner license in the state of practice.
- Experience in palliative care or related fields is preferred but not mandatory.
- Proficiency in clinical skills such as IV infusion, phlebotomy, venipuncture, and suturing is highly desirable.
- Strong interpersonal skills with the ability to provide empathetic support to patients and families during challenging times.
- Knowledge of infectious disease management and the ability to implement appropriate interventions.
- Ability to work effectively within a multidisciplinary team environment while maintaining professional standards of practice.

Job Type: Part-time
Pay: $45.00 - $48.00 per hour
Expected hours: 25 – 30 per week
