We are seeking a Clinical Navigator to serve as a behavioral healthcare subject matter expert on Traumatic Brain Injury (TBI), Post Traumatic Stress Disorder (PTSD), Dementia/Alzheimer’s, and Serious Mental Illness (SMI) for Region 5.
Day to day responsibilities include coordinating training and learning experiences, assistance with linkage and admission to treatment services, clinical consultation, marketing, and help to plan, develop, and implement treatment related services and supports for the BIRST and Gero programs.
General Responsibilities and Duties for Traumatic Brain Injury Navigator:
- Responsible for consumer assessment, service linkage, coordination
- Identify patient needs and provide referrals for individuals, caregivers, and professionals.
- Monitor service delivery through contacts with consumers receiving services, family
- Collaborate with other professionals to assess client needs or treatment planning
- Maintains sensitive, private health information in accordance to HIPAA standards and regulations.
Qualifications and Skills for Traumatic Brain Injury Navigator:
- Understanding of dementia, serious mental illness, and traumatic brain injuries, and its impact on physical and psychological states and relationships.
- Ability to establish and maintain effective working relationships with employees and community partners
- Ability to research and effectively share knowledge with colleagues and the general public.
- Some knowledge of military culture as well as severe trauma.
- Ability to organize and prioritize with minimal direction and maintain a well-organized work environment.
- Ability to problem-solve, use good judgement, and deal with diverse and difficult personalities.
- Ability to work under moderate to high measures of stress.
- Excellent written and verbal communication skills.
- Strong computer skills to include Microsoft Office Suite (Word, PowerPoint, Excel, Outlook)
- BA/BS from an accredited college or university with major course experience in Psychology, Social Work, Gerontology or related field
- QMHP or QIDP preferred
- At least three (3) years of experience in Human Services.
