What are the responsibilities and job description for the Social Services Designee position at Westminster Village North?
This position's hours will be from 11:00 a.m. to 7:00 p.m.
Essential Functions for social services.
- Conduct interviews with residents, staff and family members as appropriate to complete the following sections of the MDS: Hearing, Speech and Vision (B) Cognitive Patterns (C) Mood (D) Behavior (E) and participation in Assessment and Goal Setting (Q)
- Provide supporting documentation through review of chart information for current psychological or neurological diagnoses, psychotropic medications, and behavior issues during the observation period.
- Collect social history information for new residents.
- Write progress notes documenting specifics of MDS interview.
- Help ensure that supplies are maintained at adequate levels; recommend equipment and supply needs.
- Complete necessary forms and reports, submitting them to the administration as required.
Essential Functions for MDS.
- Complete pain interview (J) and preferences for customary routine and activities (F) sections of MDS.
- Daily, gather discharged charts and deliver to the MDS office. Make sure that Medical Records receives them after the MDS sections are completed and there are no further needs.
- Help ensure that supplies are maintained at adequate levels; recommend equipment and supply needs.
- Complete necessary forms and reports, submitting them to the administration as required.
Other Functions
- Attend meetings and participate in committees as requested.
- Attend and participate in workshops and seminars to enhance skills as well as to maintain a professional status.
- Perform other related duties and responsibilities as directed.
Education/Experience
- Must possess a Social Services Designee Certificate.
- At least One (1) year working closely with the elderly population.
