DUTIES AND RESPONSIBILITIES
- Completes a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
- Participates in the development of a documented, interdisciplinary plan of care for each resident that identifies the psychosocial needs, the goals to be accomplished to meet those needs, and the appropriate social services interventions.
- Ensures or provides interventions to assist residents in coping with their transition and adjustment to a nursing unit, including their social, emotional, and psychological needs.
- Provides support and education to residents/family members/significant others to assist in their understanding of the process.
- Coordinates additional services, and support with social service agencies for the resident, family members and others when the service is unavailable within the community.
- Provides information to residents and/or families about Medicare, Medicaid and other financial assistance programs available.
- Coordinates and encourages life enrichment activities as appropriate for each resident.
- Ensures all social services interventions are documented timely, accurately, and in accordance with state and federal regulations.
- Develops a plan of correction and/or implements quality assurance initiatives for social services concerns or deficiencies identified during a department of health inspection or otherwise.
- Develops, implements, and maintains an ongoing quality assurance program for social services.
- Educates residents, employees, and families/significant others regarding resident rights and responsibilities, effective problem solving and the health and social services that are available to them within the community, and upon discharge.
- Serves as the resident advocate and/or grievance official for the department and manages the grievance process by documenting and attempting to resolve grievances. Conducts prompt, thorough, and confidential investigations in accordance with state and federal requirements and dependent upon the type of allegation.
- Coordinates the resident discharge planning process and makes referrals for appropriate home care and other services prior to the resident’s return to the community.
- Keeps abreast of current state and federal regulations and professional standards.
- Submits reports/recommendations to the executive director concerning the operations and budget for the social service department.
- Performs supervisory functions:
1. Participates in the hiring, onboarding, and orientation process for new employees.
2. Promptly and thoroughly addresses employee disciplinary action and terminations in coordination with human resources.
3. Provides training, coaching and supervision to employes.
4. Provides timely and accurate performance management evaluations and participates in goal setting for the employee, in accordance with the organization’s established policies and procedures.
5. Prepares, implements, and monitors staffing schedules and work assignments.
6. Administers department and organizational policies and procedures in a fair and consistent manner.
7. Completed the payroll process in a timely and accurate manner.
- Performs other duties as assigned.
JOB QUALIFICATIONS
Required: a bachelor’s or advanced degree, or both, in social work or in a human services field including Sociology, Gerontology, Special Education, Rehab counseling, and/or Psychology.
Preferred: Indiana board certification in social work with a minimum of five (5) years’ experience in a health care setting working directly with individuals.
