Job Type: Full-time (Monday–Friday, 8:00 AM – 5:00 PM)

Position Summary: The Wound Care/Admissions Nurse plays a key role in both the care and welcoming of new residents to our community. This full-time position includes:
- Admissions Responsibilities (60% of role):
o Greet and welcome new residents as they move in, ensuring a smooth and welcoming transition into the community.
o Conduct comprehensive assessments of new residents to gather medical and personal history to assess care needs.
o Meet with new residents and their families to introduce the facility, explaining services and community offerings.
o Ensure all required documentation is completed accurately and promptly, including consent forms, medical history, and insurance paperwork.
o Collaborate with other departments to prepare the resident's room and ensure they have all necessary accommodations.
o Serve as the primary point of contact for incoming residents, addressing questions and concerns regarding their transition.
o Coordinate with IDT to ensure a seamless and positive admission experience.
- Wound Care Responsibilities (40% of role):
o Assess and treat residents with wounds, pressure ulcers, and other skin conditions.
o Develop individualized wound care plans for residents, in consultation with physicians and other healthcare providers.
o Regularly monitor and document wound healing progress, adjusting care plans as necessary.
o Administer wound care treatments, including dressing changes and debridement procedures.
o Educate residents and their families on wound care best practices to encourage healing and prevent further complications.
o Maintain accurate and thorough medical records related to wound care treatments and progress.
o Stay current on wound care best practices, techniques, and new developments.
Qualifications:
- Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current, active state licensure.
- At least 3-4 years of experience in nursing, preferably in long-term care, rehabilitation, or a similar healthcare setting.
- Wound care certification is a plus (e.g., CWCN, WCC, or other related certifications).
- Strong clinical skills with experience in wound care management and treatment.
- Excellent communication and interpersonal skills, with the ability to work well with residents, families, and staff members.
- Strong organizational skills and attention to detail, with the ability to manage multiple tasks simultaneously.
- Ability to maintain confidentiality and adhere to HIPAA guidelines.
