Willow Glen Care has locations throughout Northern California designed to serve multiple levels of care and behavioral health programming for adults and the elderly with mental illness. Willow Glen and its associated programs are designed to coordinate care with county mental health agencies to facilitate referrals, care and treatment, discharge planning and placement.
The program:
Hyperion CRTF is a 10-bed facility in a newly-renovated building providing 24/7 assistance to adults struggling with behavioral health crises and provides crisis stabilization as an alternative to emergency room/hospitalization. While residing at the CRTF, clients can engage with comprehensive programs assisting them in accessing appropriate services and treatment, utilizing peer support, medication management, and self-directed goal setting, in a safe and home-like environment.
The position:
Under the direction of the House Manager, the House Nurse provides medication oversight for the 10 residents of the program, and trains direct-care staff in medication management. In addition to overseeing facility medications and overseeing basic nursing duties, the Nursing Supervisor will be an important part of the leadership team at the facility and will help with all aspects of staff management. The Nursing Supervisor and Program Director will work closely with clients and employees to ensure programmatic success.
Essential Duties and Responsibilities of the position:
- Oversees direct-care staff and assists with all aspects of staff management.
- Works closely with the Program Director to hire, onboard, and train employees.
- Supports clients with the self-administration of prescribed medication and documents such in a timely manner.
- Identifies effectiveness of PRN medication administered and documents in a timely manner.
- Monitor and document clients' intake of psychiatric medications and ensure that medications are handled and stored in accordance with established program procedures and licensing regulations; regularly review medication logs to ensure that they are accurate and complete (when assigned to the medication room).
- Facilitates the development of therapeutic relationships between staff and clients.
- Documents observations, patient response to treatment, and plan for care to continue stabilization.
- Provides early intervention as a necessary component of crisis management in the milieu.
- Able to direct the crisis counselor staff in the care of the client.
- Notifies House Manager and Medical Director of potential adverse events and assists in problem solving.
- Effects consistently timely and thorough communication with medical staff.
- Attends treatment team as an active participant, when required.
- Assures that protected health information is managed per HIPAA requirements.
- Demonstrates milieu management techniques with a variety of client populations and age groups.
- An active member of the clinical treatment team.
- Provide accurate, timely, and complete program and client information transfer to other staff as they begin their shift.
- Demonstrates understanding of the job functions.
- Provides assistance and care to clients as is expected of the position, department, or facility or as listed above in job description.
- Demonstrates a high level of awareness and continual observation of clients at all times when in client areas and provides intervention or reports to supervisor, as appropriate.
- Identifies early signs and symptoms of potential crisis or conflict with clients and intervenes or reports to appropriate supervisor as required by the position.
- Provides crisis intervention and/or de-escalations as necessary or reports to appropriate supervisor for intervention.
- Represents and conducts self within the agency and the general public in a manner that promotes the dignity and respect of adults with mental illness.
NOTE: This position may being on-call for emergency and after-hours management.
Requirements and Qualifications of the Nursing Supervisor:
- A High School Diploma or GED is required.
- Possession of a valid license as an LVN, or LPT issued by the State of California.
- Must be able to receive and maintain a criminal records clearance, as required by Title 22, other licensing regulations, and Willow Glen practices.
- This position requires frequent driving. A valid California driver's license, reliable personal vehicle, current personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies are all essential job requirements.
- Must be able to pass post-offer, pre-employment medical and drug tests as required under State Community Care Licensing regulations and/or agency policies.
- Must obtain and maintain First Aid and CPR certification
Physical, Environmental and Mental Requirements:
- Physical: Occasionally required to push/pull objects up to 50 lbs., and to lift/carry objects up to 25 lbs. Frequently required to perform moderately difficult manipulative tasks such as typing, writing, etc. Must be able to walk, stand, and sit for extended periods. May be required to regularly go up and down stairs and/or staircases.
- Sensory: Frequently required to read documents, written reports, and plans. Must be able to distinguish normal sounds with some background noise, as in answering the phone, interacting with residents and staff, etc. Must be able to speak clearly and understand/be understood using the English language.
- Cognitive: Frequently required to concentrate on moderate detail with constant interruption. Must be able to attend to a task/function for 20-45 minutes at a time. Frequently required to understand and relate to specific ideas, several at a time. Must be able to remember multiple tasks/assignments given to self and others over a period of several days or weeks.
- Psychological: Potential exposure to loud tones, offensive, intimidating, or threatening language or behaviors, in the client care environment.
- Environmental Conditions: Frequent exposure to varied office and mixed (residential/office) environments. Occasional exposure to toxins and poisonous substances, dust, and loud noises. Potential exposure to bodily fluids, processes, and odors.
- Equipment: Frequently required to use a computer, phone, copy, and fax machine. May need to use other equipment, based on department need and with appropriate training.
