Job Description Job Description Wilshire Community Services is looking for a Case Manager for our Older Adult Full-Service Partnership program. The ideal candidate would have experience working with older adults with mental illness. We are looking for an experienced case manager that can work compassionately with that population and will work well within our team that provides wrap around services.Overview of the PositionThe Older Adult FSP case Manager works as part of the FSP ream at Wilshire Community Services. The Case Manager will be responsible for working in close collaboration with the FSP Clinician to support clients as they strive to reach their treatment plan goals. The Case Manager provides case management as needed to include; connection to community resources, assistance with transportation, assistance with securing basic needs (food, clothing, and shelter), and providing coaching / education To FSP clients.Core Performance Requirements and Essential Functions of the Job Summary : Support the development and attainment of clients’ wellness goals; including : securing appropriate housing, development of a support system, increasing socialization skills, participating in recreational activities, etc.Assist with and teach activities of daily living; such as : meal planning and preparation, personal hygiene and budgeting.Support and assists with the maintenance of clients’ personal health; including the attainment and follow through with medical, psychiatric, and dental care.Document in an accurate and timely manner any treatment planning and / or interventions in accordance with the policies of Wilshire Community Services, SLO County Behavioral Health and MediCal.Be an active participant in FSP Team Meetings by contributing to the development of individual treatment and behavior plans.Demonstrates knowledge of psychological diagnosis and presentation of behaviors.Respect clients’ / co-workers’ rights and confidentiality as directed by law and pursuant to regulations and agency policies, and the principles of ethics.Is responsive to office needs, i.e., helps others as requested by the Administrator of Wilshire Community Services and functions as an active member of the team.Effectively plans and organizes workflow assignments to maximize output and efficiency.Demonstrates activities to control expenses.Qualifications and Requirements1. Bachelor’s Degree in related Human services field and at least 6 months mental health experience OR at least two years mental health experience. Previous experience working with older adults is preferred.2. Ability to handle intense situations, display good judgment and decision-making skills, and remain calm and clearheaded under pressure.3. Strong interpersonal skills and oral / written communication skills; well defined organizational and time management skills. Able to deal effectively with a variety of people and personality types regardless of ethnic, racial, religious background or socio-economic level.4.
