Woodland Manor, a skilled nursing facility, is seeking a dedicated Occupational Therapist (OT) to join our rehabilitation team on a PRN basis. In this role, you will provide therapy services that enhance independence, improve patient functioning, and support residents in achieving their best quality of life.
Responsibilities :
- Implement individualized treatment plans as established by the primary Occupational Therapist.
- Collaborate with the rehabilitation team to develop therapy programs tailored to each patient’s needs and capabilities.
- Educate patients, families, and caregivers on techniques to support therapy goals.
- Conduct evaluations to gather data for treatment planning and establish both short-term and long-term goals.
