POSITION TITLE: Rapid Rehousing Case Manager
RESPONSIBLE TO: Housing Stabilization Coordinator
RESPONSIBLE FOR: This is a full-time, non-exempt position. The Rapid Rehousing Case Manager plays a pivotal role in efficiently coordinating rapid rehousing programs for individuals and families experiencing homelessness. This multifaceted role entails close collaboration with individuals and families requiring intensive case management, provision of comprehensive wrap-around support, and guidance through the available resources in the Yamhill County area. Key responsibilities encompass situational analysis, service application, intake and assessment, and ongoing case management, all aimed at ensuring housing placement and stability for individuals and families experiencing homelessness and those in need in our community.
WORK ENVIRONMENT: This position involves close interaction with the unhoused population, both in an office setting and where persons experiencing homelessness may congregate.
ESSENTIAL DUTIES:
1. Create a positive team atmosphere providing quality service to clients, staff and volunteers through active listening, positive problem solving, and timely responses.
2. Assess client needs and make appropriate referrals to agency programs or community resources.
3. Provide case management for clients in agency programs, including assistance with service applications, goal setting, advocacy, and referrals to partner agency services.
4. Provide timely and quality assessment, screening, and connection to services for individuals and families identified as meeting the qualifications to receive rapid-rehousing assistance.
5. Utilize Triage Case Management techniques, employing diversion tactics by referring to other local partners.
6. Provides on-going intensive Case Management to households including but not limited to: addressing housing barriers, creation and implementation of housing stability plan, and brainstorm intervention strategies to address individual needs or challenges that may arise for assigned participants.
7. Provide or connect households to basic need supports (food, hygiene products, clothes), counseling, jobs assistance, legal and case management.
8. Provide referrals, resource connection and education to meet basic needs and support the housing plans of persons experiencing homelessness, including but not limited to: Rent-Well classes and support, regular collaboration with partners such as mental health, public health, Northwest Seniors, and the Housing Authority of Yamhill County.
9. Accurately and consistently enter data into HMIS Service Point/Well-Sky in order to track outcomes.
10. Maintain all files, documentation, and case notes to meet monitoring standards.
11. Ensure timely reporting, accurate data input, and compliance with grant standards. Assist with data for monitoring site visits.
12. Assist with agency activities related to homelessness, such as the Homeless Count and Community Connect.
13. Complete specialized trainings, including De-Escalation, Trauma-Informed Care, Coordinated Entry, Non-Violent Communication, Harm Reduction, Fair Housing, Domestic Violence Support, Mental Health First Aid, and SOAR.
14. Collaborate with other agencies, businesses, and volunteers to optimize resource utilization.
15. Perform local, in-county driving to client and partner agency locations.
16. Undertake other duties as assigned.
KNOWLEDGE, SKILLS, & ABILITIES:
- Bachelor's degree in sociology or equivalent experience
- Proficiency in computer usage, software programs, and databases
- Strong verbal and written communication skills
- Effective problem-solving abilities and teamwork
- Ability to maintain confidentiality of client and organizational information
- Capability to work independently on multiple projects in a timely manner
- Aptitude for working with diverse individuals, organizations, and situations
- Ability to meet insurance requirements on driving record and pass background and drug screen
PHYSICAL DEMANDS: The physical demands of this job may require sitting, standing, kneeling, walking, and/or bending for extended periods. The Case Manager may also need to lift and/or move up to 20 pounds. Case Manager will be subjected to outside environmental conditions while conducting various levels of Case Management to homeless neighbors. In addition to working in a typical office environment, the Case Manager will be required to work in areas where homeless persons may congregate.
Job Type: Full-time
Pay: $24.67 per hour
