Job Summary
The Registered Nurse Case Manager plays a critical role in coordinating patient care and ensuring that individuals receive the appropriate medical services throughout their healthcare journey. This position involves working closely with patients, families, and healthcare teams to develop and implement effective care plans, particularly for those with complex medical needs. Duties
- Conduct thorough assessments of patients to determine their healthcare needs and develop personalized care plans.
- Coordinate with multidisciplinary teams to facilitate patient care across various settings, including outpatient services and hospital medicine.
- Manage discharge planning processes to ensure smooth transitions from hospital to home or other care facilities.
- Maintain accurate medical records and documentation of patient interactions, treatments, and progress.
- Perform diagnostic evaluations to identify patient needs and recommend appropriate interventions.
- Provide education and support to patients and families regarding treatment options, disease management, and preventive care.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Collaborate with healthcare providers in a Level I Trauma Center environment to ensure comprehensive care for trauma patients.
Skills
- Strong background in patient care with experience in infectious disease management.
- Proficient in handling cases related to Pediatric Intensive Care Unit (PICU) as well as geriatric populations.
- Knowledgeable in outpatient care practices and hospital medicine protocols.
- Excellent discharge planning skills with the ability to navigate complex healthcare systems.
- Familiarity with maintaining medical records and ensuring compliance with healthcare regulations.
- Strong diagnostic evaluation skills to assess patient conditions effectively.
- Ability to communicate clearly and compassionately with patients, families, and healthcare professionals.
- Proven organizational skills to manage multiple cases effectively while prioritizing patient needs.
Job Type: Contract
Pay: $2,611.00 - $2,950.00 per week
