POSITION REPORTS TO: Field Supervisor (Supervisor-Enroll/Elig)
JOB OVERVIEW:
The Enrollment Case Manager will be scheduled (in most instances) throughout the day with appointments that have a travel time of 45 minutes or less from their prior location, usually within their assigned county. There may be instances where ECM’s are expected to, and scheduled to, travel outside of this radius as dictated by business needs. ECM’s work primarily in applicants' places of residence. They will conduct intake visits for waiver applicants and acts as a facilitator for the eligibility determination.
JOB DESCRIPTION AND RESPONSIBILITIES:
- Traveling within & outside of assigned county, that will include working primarily in applicants' places of residence (home, nursing facility, shelter, etc…) in various conditions of cleanliness
- Conduct & complete an intake visit for program applicants to facilitate a successful application for the eligibility determination process. This process includes completing a structured assessment, education on this & other relevant programs, coordination amongst outside departments, completing a COMPASS or PA600L, provision of relevant resources & ensuring all documents are accurate & completed to the extent the Applicant is capable of participating. Documenting the appropriate outcomes & important information from each appointment/correspondence.
-Complete follow up visitation as requested by an Applicant for what is determined by the Applicants’ County Assistance Office that is needed to meet the eligibility requirements for the waiver program.
- When there is not a need in the field, this position will have other duties assigned to complete outreach to Applicants & or relevant supports/services to assist in meeting eligibility requirements or work w/in the program database to support program needs.
- This position is expected to represent the project & it’s mission & will act in a manner that is ethical & Applicant focused. All information & materials presented will be done in an unbiased/objective manner. This includes; presenting the overview of the waiver program, the LIFE, CHC, OBRA, EPSTD, OLTL, AAA, & CAOs, review of the applicants rights and responsibilities, as well as making no recommendations to personal choice of MCO or SPC.
- Assists with developing relationships with community-based organizations, advocates and stakeholders to gather feedback on improving the application process and removing barriers to enrolling in waiver programs
- Fosters and maintains positive working relationships with parties serving our mutual consumers, including physicians, County Assistance Offices, providers, nursing homes and rehabilitation facilities
- Facilitates the efficiency of the application process, ensuring any barriers to this process are escalated for review or resolved at each appointment
- Attends regularly scheduled meetings (supervision, team mtg, trainings, all staff mtgs…)
- Have the ability work independently to manage your schedule, communicate effectively w/ all points of contact in a workday, provide excellent customer service, possess consistent time mgmt. skills, implement organizational skills, has the ability to accept & adapt to a variety of environments
- Understand the responsibilities of a Mandated reporter & execute those actions as needed
- Proficient technology skills that include; experience in the Microsoft Office Suite (Word, Excel, Teams, SharePoint), basic typing skills for reporting & data input, use of electronic documentation, scanning documents & use of laptop & smart phone to accomplish all of the above functions
EDUCATION/EXPERIENCE: Enrollment Case Managers must satisfy one of the following qualifications:
1. A minimum of one year experience in public or private social work; or a minimum of two years of case work experience including one year of experience performing assessments of individuals functional ability to determine the need for institutional or community-based services; and a Bachelor’s degree, with or supplemented by a minimum of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social science; Or a Bachelor’s degree with a social welfare major; Or any equivalent combination of experience or training with successful completion of a minimum of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social science.

a. One-year experience in a local AAA office may be substituted for one-year assessment experience.
Relevant experience may include: Coordinating assigned services as part of an individual’s treatment plans; Teaching individuals living skills; Aiding in therapeutic activities; Providing socialization opportunities for individuals.
Relevant experience DOES NOT include: Providing hands-on personal care for people with disabilities or individuals over 60 years of age; Maintenance of the individual’s home, room or environment; Aiding in adapting the physical facilities of the individual’s home.
Other desired skills include: Excellent organizational, written and verbal communication skills; Ability to interact courteously and effectively with a variety of audiences, including the disability community; Ability to handle pressure and perform multiple tasks; Ability to work independently; Experience entering data into automated information systems; Knowledge of local health care networks and providers; Ability to maintain cultural sensitivity. Must be willing to travel to applicants' places of residence throughout assigned region. Occasional evening and weekend hours required. Employment is contingent upon successful clearance of criminal and child abuse background checks and an absence of affiliations with Office of Long-Term Living (OLTL) providers
Pay: $20.50 Monolingual or $21.50 Bilingual (Spanish)
