SUMMARY:

Responsible for the daily operations of the hospice program including, referral management, coordination of admissions, discharges and change of levels of care, management of ancillary vendors, maintaining patient records, coordination and preparation of meetings, and clerical functions. The Patient Care Coordinator is responsible for maintaining a high level of customer service with the patients, family members, provider, vendors and referral sources.

QUALIFICATIONS:

Education and/or Experience:

Licensed Medical Assistant or Licensed Practical Nurse with at least 2 years of medical experience and training. Hospice experience preferred but not required

Current licensure in the state of NH

Previous experience with an electronic medical records system.

Proficient in Microsoft Office software

Effective social interaction, communication and organizational skills and the ability to perform a variety of tasks efficiently.

Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.

High level of customer service experience.

Licenses/Insurance:

Access to operable, insured motor vehicle and valid driver's license with acceptable record of safe driving practices.

ESSENTIAL FUNCTIONS:

  • Assists scheduling in coordinating Hospice admissions, routine visits, pronouncements, supervisory visits and recertification's. Brings any scheduling problems to the attention of the supervisor immediately.
  • Ensures seamless and timely transition of patients to the Hospice program by coordinating all referrals with the Director of Operations and Clinical Services.
  • Creates Hospice charts in the EMR and establishes each patient with agency vendors diligently and efficiently, ensuring there are no errors with dates or insurance information.
  • Coordinates ordering and delivery of DME, supplies and medications.
  • Collaborates with pharmacy, DME and supply vendors to ensure timely delivery of all supplies. Communicates with the patient and family to ensure they are involved and aware of these processes and addresses their needs in a thoughtful manner.
  • Serves as a resource to all patients and clinicians for issues, concerns and first line triage.
  • Documents and reports to the Director of Clinical Services and/or supervising nurse when there is a change in client status, changes in service requirements, change in employee assigned to case, and reports any unsatisfactory performance.
  • Tracks and coordinates admissions, discharges, IDTs, certification and re-certification on all Hospice patients
  • Coordinates Palliative Care referrals to Hospice as needed with the Palliative Care Coordinator
  • Responsible for assisting with audits of patient information
  • Creates bereavement charts and ensures Social Work has information needed to perform bereavement visits.
  • Demonstrates good organizational skills and exceptional customer service skills
  • Uses equipment and supplies in an appropriate and cost effective manner
  • Acknowledges that all information related to employees, clients and their families, and NCHHHA business practices is confidential
  • Practices proper body mechanics and observes safety measures
  • Adheres to all safety policies including reporting/ removing hazards immediately
  • Demonstrates knowledge and compliance with infection control policies
  • Follows instructions, responds to management direction
  • Works cooperatively with other NCHHHA personnel
  • Complies with NCHHHA's Standard of Conduct
  • Required completion of time sheets and other records in accordance with agency policy and State and Federal regulations
  • Other duties as assigned
