SUMMARY:

The Hospice and Palliative Care Spiritual Counselor will be a member of an experienced hospice care team that provides assessments, education and symptom management related to end of life care. The ideal candidate will have prior experience as a Spiritual Counselor in hospice or palliative care and will be responsible for developing and adhering to the client's spiritual care plan.

QUALIFICATIONS:

Education and/or Experience:

Graduate of accredited seminary school of theology, and/or certificate of a seminary program completion, or equivalent theological/social science degree. Experience preferred in pastoral care, social work, or counseling.

Hospice experience preferred but not required.

Comfortable in interfaith setting.

Ability to accept different lifestyles, cultures, beliefs, and values.

Previous experience with an electronic medical records system.

Proficient in Microsoft Office software; emailing, record keeping

Effective social interaction, communication and organizational skills and the ability to perform a variety of tasks efficiently.

Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.

High level of customer service experience.

Licenses/Insurance:

Access to operable, insured motor vehicle and valid driver's license with acceptable record of safe driving practices.

ESSENTIAL FUNCTIONS:

  • Provide direct spiritual care to patients and families/caregivers.
  • Complete assessment of spiritual needs to patients and families/caregivers and render appropriate services in a timey manner.
  • Maintain electronic medical records of spiritual care services provided to patients and families/caregivers in a timely manner.
  • Initiate and update spiritual plan of care by charting clinical problems, goals and interventions in the comprehensive assessment form and plan of care.
  • Attend and contribute to patient care conference and interdisciplinary group meetings.
  • Provide clinical consultation and education to the interdisciplinary group on spiritual care.
  • Serve as a liaison to community-based chaplains, clergy and spiritual counselors.
  • Officiate funerals or memorial services for families, as requested.
  • Participate in Agency memorial events.
  • Demonstrates good organizational skills and exceptional customer service skills.
  • Uses equipment and supplies in an appropriate and cost effective manner.
  • Acknowledges that all information related to employees, clients and their families, and NCHHHA business practices is confidential.
  • Practices proper body mechanics and observes safety measures.
  • Adheres to all safety policies including reporting/ removing hazards immediately.
  • Demonstrates knowledge and compliance with infection control policies.
  • Follows instructions, responds to management direction.
  • Works cooperatively with other NCHHHA personnel.
  • Complies with NCHHHA's Standard of Conduct.
  • Required completion of time sheets and other records in accordance with agency policy and State and Federal regulations.
  • Other duties as assigned
