The employee may be working towards board certification as part of a timeline mutually agreed upon by the employee and supervisor Licensure as a Pharmacist in New York State required RESPONSIBILITIES Possess a broad scope and depth of pharmacotherapy knowledge and clinical skills. Independently responsible for complex decision-making, such as drug therapy selection Works in collaboration with others across the Health System providing patient care and medication consultation services Able to develop, recommend, and monitor therapeutic plans Function on Health System multidisciplinary teams in the development of protocols, order sets, and critical pathways to optimize patient care. Works with pharmacy management and specialty departments to implement policies and procedures to ensure compliance with all applicable regulations Detect, prevent, monitor, document, and report adverse drug events and medication errors Respond to patient care urgencies/emergencies within your area of expertise EDUCATION: BS: Pharmacy (Required) LICENSES / CERTIFICATIONS: LPharm - Licensed Pharmacist - New York State Education Department (NYSED) PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. PAY RANGE: $126,400.00 - $167,500.00 CITY: Clifton Springs POSTAL CODE: 14432 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
