This is a home health visit position located in patient's homes throughout Chicagoland and surrounding suburbs.

EHR- AXXESS

Duties :

  • Licensed Practical Nurse administers medications and treatments as prescribed by physicians
  • Licensed Practical Nurse collaborates with healthcare professionals to plan, implement, and evaluate patient care plans
  • Licensed Practical Nurse provides care at patient's place of residence

Experience :

  • Must have a valid LPN license in the state of practice, Driver's license, & proof of insurance
  • Previous experience in home health or hospital setting is preferred, at least a year
  • Familiarity with EHR systems and OASIS documentation

    • Pay : From $55.00 per visit

    Expected hours : 1 – 20 per week
