This is a union position.    The Prototypes Alternatives to Incarceration (ATI), a program of HealthRIGHT 360, is part of a Los Angeles County-wide initiative, seeks to reimagine the justice system by diverting individuals arrested for misdemeanors and non-serious, non-violent felonies from criminal charges to community-based support services. ATI addresses key factors leading to incarceration, including homelessness, mental health, addiction, and lack of employment opportunities. Through collaboration with law enforcement and district attorneys, ATI is working to build a sustainable network of providers that enhances the community's ability to support those in need.   Under the guidance of the Program Manager, the Case Manager for Alternatives to Incarceration will connect clients to essential community-based services to create a solid foundation of support and prevent incarceration. This includes coordinating access to housing, healthcare, substance abuse treatment, and social services. The Case Manager will enhance the continuum of care by providing client-centered, supportive services. As a field-based position, the Case Manager will meet clients in various community settings, including their homes, shelters, or residential facilities. This role will be utilizing trauma-informed practices, restorative justice, harm reduction strategies, resilience-building, and community safety planning to reduce recidivism.   KEY RESPONSIBILITIES  Case Management Refer and link to mental health and addiction treatment, including assistance with housing, benefits, employment, education, transportation, childcare, medical, and other services. Provide housing navigation and utilize short-term housing flex funds. Collect all pre- and post-surveys and outcome measures. Engage clients in ongoing follow-up and motivate them to support their goals. Empathetically support and provide feedback that is trauma-informed and culturally competent. Conduct engagement and enrollment of clients. Obtain all appropriate consents and authorizations, including any County-required forms. Serve as the central point of contact for the clients through which care and services are coordinated. Collaboratively develop and implement an individualized care plan within 30 days of program enrollment through needs assessment, identify strengths, and help clients define goals. Maintain a caseload of no more than 30 clients.  Meet clients at least once a week in person for the duration of their enrollment. Exceptions may be granted if meeting in person is disruptive to the client.     Field-Based Responsibilities Meet clients at their physical locations and cater to their individual needs. Conduct case management activities in locations most attainable and desirable for clients. Have access to office space for case management activities and attend required training and meetings as needed. Submit mileage for field-based case management activities for reimbursement.   Administration Maintain all documentation at a high standard, completed in a timely manner, and accurate. Maintain confidentiality in compliance with agency, HIPAA, and 42-CFR standards. Interface with a multidisciplinary team in treatment planning and service delivery. Communicate with prosecuting agencies within 48 hours if a client leaves the program. Adhere to all policies and procedures established by HealthRIGHT 360's Health & Safety program, including but not limited to participating in emergency preparedness drills, reporting any potential safety hazards to the appropriate party, and following proper protocols for response to emergency situations. Complete assigned training in a timely manner. Attend and participate in meetings.   QUALIFICATIONS Education and Experience Option I: High school diploma or equivalent. At least three years’ experience providing case management with justice-involved individuals and families. Option II: Bachelor’s degree in related field. At least two years’ experience providing case management with justice-involved individuals and families. Experience in working with families from low SES communities, diverse cultural ethnic, and linguistic backgrounds. Experience working successfully with issues of substance abuse, mental health, or co-occurring disorders, and other potential barriers to economic self-sufficiency.   Certification and Licensure Valid registration or certification as a substance abuse counselor from a certifying agency (CCAPP, CAADE, CADTP) is preferred. Current First Aid and CPR certification or ability to obtain within 30 days of hire. Valid California driver’s license and access to registered and insured transportation.   Background Check and Other Requirements Must complete a background check and livescan fingerprinting Must be capable of meeting program and funder requirements.
