Prototypes’ Outpatient mental health services provides comprehensive mental health services for men, women, and children. Our continuum of care ranges from prevention and early intervention to intensive outpatient services for those with more serious and chronic conditions, including Full-Service Partnership (FSP) services. Staff provide services at our Prototypes sites and in the community. These services can include individual and group services, medication support, case management, and coordination with other healthcare services.
The Case Manager II maintains a caseload and provides care by linking the client with appropriate services to address specific needs such as: benefits, mental health, physical health, employment, probation, DCFS, employment, housing, community resources, outpatient substance use disorder services, and aftercare. The Case Manager II offers support services to clients to increase self-efficacy, self-advocacy, basic life skills, coping strategies, self-management of biopsychosocial needs, benefits and resources, and reintegration into the community. The Case Manager II works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360’s philosophy, goals, policies, mission, and vision. Services may be provided in: outpatient office, residential treatment, or the field, based on client and program need.
KEY RESPONSIBILITIES
Direct Service:
Assists the client with intake by completing case management assessment, completes all releases of information, and enters financial and benefit information into EHR.
Adds problems to the Problem List within scope, monitors client progress, and closes problems as appropriate.
Supports the client in applying for Medi-Cal or transferring Medi-Cal to LA County when appropriate.
Assess the client’s case management needs and completes all releases of information.
Ensures clients on caseload have a PCP, a release to speak with the PCP, and at least one outreach attempt to the PCP has been documented within the first 30 days of admission.
Connects the client to benefits, healthcare services, employment, housing, community resources, outpatient substance use services, and other benefits/resources available to the client.
Coordinates communication and external service linkage including assisting with scheduling appointments, communicating with probation, scheduling child visits, communicating with DCFS, obtaining all court minute orders, providing appointment reminders for therapy and psychiatrist.
Completes the VI-SPDAT and connects the client to safe housing options.
Collaborates with clients to create and work towards rehabilitation goals.
Prepares the client for job search by assisting with resume writing and job search preparation. Research necessity information to provide appropriate referrals.
Seeks clinical consultation throughout the treatment episode, especially when encountering new and/or high-risk clinical circumstances.
Completes client UA tests and documents test results (when assigned).
Maintains frequent communication with the treatment team and engages in regular consults, clinical team and case conferences, and plan development meetings.
Delivers services in outpatient, residential, or field depending on assignments and/or client need(s), including before and after business hours.
Responsible for treatment oversight of case management services for all clients on caseload.
Meets expected client care hours (productivity expectation is 25 client care hours per week/ 1500 minutes weekly).
Child Specific
Provide support for parents and caregivers to strengthen the family members’ network of relationships and community resources
Provide field-based services as required by program
Collaborate with school staff, and advocate and participate in IEP or 504 plan meetings as needed
FSP-Specific
Complete and submit rent invoices/ flex fund request/ and vendor check requests as needed.
Complete KEC/OMA’s at scheduled due date.
Participate in Outreach and Engagement Process (including commuting into the field as needed).
CRP-Specific
Outreach and engagement for referred individuals and clients struggling to meet probation mandates (Including commuting into the field as needed).
Complete weekly outreach and engagement reports.
Provide jail in-reach and post-release case management as needed. (Services may take place in the office, jails, and probation HUB’s)
Collaborate with clients and their families to support reintegration into the community.
Coordination and collaboration with law enforcement to promote compliance and increase the likelihood of success of the client.
Assist with the development of groups and activities that meet the needs of the clients served.
Offer peer support to clients when needed to obtain needed community resources
Collaborate with outside agencies to help clients obtain needed community resources (housing, sober livings, substance abuse recovery agencies, medical facilities, employment/education agencies, DPSS, DMV or other needed resources)
Clinical Documentation:
Write and complete all progress notes within 72 hours of service delivery.
Maintains compliance with due dates for all clinical documentation.
Writes client progress letters and court reports and obtains final approval from appropriate supervisor.
Documentation must maintain in compliance with agency policy and procedures, HIPAA, 42-CFR, DMH, and SAPC standards.
Completes all assigned Peer Reviews (Chart Audits) within a timely matter.
Training, Supervision and Quality Improvement:
Attends and actively participates in assigned supervision (individual and/or group).
Actively participates in agency and team meetings.
Participates in training opportunities.
Communicates collaboratively with all members of the behavioral health team, including medical, mental health, psychiatry, substance use disorder, and other staff.
Completes all assigned trainings and Relias trainings in a timely manner.
Professionalism and Other Duties:
Arrange work schedule in accordance with the agency’s needs.
Attend regular staff meetings and other meetings as assigned.
Reads emails daily and responds when appropriate.
Transports clients as needed.
Works collaboratively with other members of the treatment team including the Clinical Therapist, Counselor, Substance Use Treatment Team, Medical provider(s), and Psychiatrist.
QUALIFICATIONS
Education, Certification, and Experience
Required:
Bachelor’s Degree from an accredited university OR high school diploma with AOD Certification OR AOD Registration with 2 years working in human services field.
Experience working with clients experiencing acute withdrawal from substances.
Knowledge of and experience with providing trauma informed services.
Experience delivering evidence-based practices preferred.
Desired:
Preferably a minimum of one year experience in the human service field and/or demonstrated expertise in substance abuse treatment, relapse prevention, and recovery.
Background Clearance

Culturally competent and able to work with a diverse population.
Strong proficiency with Microsoft Office applications, specifically Word, Outlook and internet applications.
