Responsibilities
- Discuss barriers to support them in completing their certificate program of choice
- Discuss solutions and provide linkages to community resources
- Provide person-centered care for members to motivate and support members in achieving their educational goals
- Communicate with the Field Coordinators to ensure all necessary providers and supports related to the support of the member
- Ability to build rapport and work with people of different backgrounds
- Perform all other duties requested
- Minimum education requirement is High School diploma
- Knowledge within the field of case management
- Skilled in time management, organization, and ability to problem solve
- Ability to work flexible hours
