Referral Coordinator

Responsibilities

• 
  Complete referrals for members who request additional services for their children.
• Coordinate and facilitate in-person referral appointments with members to inform them about the referrals service.
• Oversee the benefit requests from members with special needs children.
• Act as the contact and liaison with members and pertinent business units, collect and validate confidential and sensitive documentation required by the Funds (i.e. Individualized Education Plans, doctor notes, etc.)
• Draft case notes for members and make determinations regarding the granting of special needs benefits based on the review and assessment of collected information.
• Responsible for member outreach and marketing of the referral services
• Establish rapport with organizations to maintain relationships for future referrals.
• Create summary reports on the progress of referrals for Management.
• Develop a system to respond to members in a timely manner.
• Serve as a liaison for contracted referral organizations to validate member eligibility and confirm the utilization of services provided.
• Perform additional duties and projects as assigned by management

Qualifications

• 
  Bachelor’s degree.
• Minimum of three (3) years of job-related experience in community organizing is required. It is preferred that candidates have experience supervising staff and/or volunteers or a combination of working with volunteers and community-based organizations, particularly in grassroots organizing or outreach.
• Proficient in Microsoft Office/Suite applications (Excel, Word, PowerPoint, Outlook, etc.).

• Excellent interpersonal and communication skills oral, written and listening.
• Ability to work flexible hours including some evenings and Saturdays
