The Pharmacy Care Coordinator helps manage medication therapy, improves patient outcomes, and ensures an optimal pharmacy experience by offering support in medication management, adherence, and education.
Key Responsibilities:
- Patient Medication Management:
- Coordinate the initiation and ongoing management of patient medication therapy.
- Ensure timely medication refills and resolve any issues related to prescription orders.
- Provide medication counseling to patients regarding proper use, dosage instructions, potential side effects, and storage of medications.
- Help identify medication-related problems and collaborate with pharmacists and healthcare providers to develop solutions.
- Patient Education & Adherence Support:
- Promote adherence to prescribed medication regimens by offering support and education on the importance of consistent medication use.
- Educate patients about potential drug interactions, side effects, and the proper use of over-the-counter (OTC) medications.
- Assist in creating personalized medication adherence plans and follow up with patients regularly to track their progress.
- Coordination of Care:
- Work closely with pharmacists, healthcare providers, and insurance companies to coordinate patient care and ensure that patients receive the appropriate medications.
- Collaborate with healthcare providers to adjust medication therapy and ensure optimal care for chronic or complex conditions.
- Assist in managing transitions of care, such as post-hospital discharge, to ensure patients continue receiving necessary medications.
- Insurance and Billing Assistance:
- Assist patients with insurance-related issues, including verifying coverage, processing claims, and handling medication prior authorizations.
- Resolve insurance rejections and work with insurance companies to ensure coverage of medications.
- Support patients with understanding their copays, deductibles, and any financial assistance programs available for their medications.
- Patient Advocacy:
- Act as a patient advocate by ensuring patients receive the medications they need and addressing any concerns or obstacles they face in accessing their prescriptions.
- Offer guidance on available programs that may reduce medication costs, such as discount programs, generic substitutions, or patient assistance programs.
- Address and resolve patient complaints or issues with medications or service delivery.
- Medication Therapy Management (MTM):
- Assist in the execution of Medication Therapy Management (MTM) services by providing regular medication reviews for patients.
- Identify potential medication-related problems such as drug interactions, duplicate therapy, or incorrect dosages.
- Provide recommendations to patients and healthcare providers to optimize medication use.
- Administrative Support:
- Maintain accurate and confidential patient records, ensuring all interactions, prescriptions, and medication plans are documented properly.
- Update and manage patient information in the pharmacy's management system.
- Assist in managing the workflow of prescription processing, including ensuring medications are ready for pick-up or delivery.
- Collaboration with Pharmacy Team:
- Work closely with pharmacy technicians and pharmacists to ensure that the pharmacy runs smoothly and that patients' needs are met in a timely and efficient manner.
- Participate in pharmacy team meetings to discuss patient care strategies, workflows, and operational improvements.
- Wellness and Preventative Care:
- Promote wellness and preventative care services, including immunizations, health screenings, and lifestyle recommendations.
- Provide resources and education on health topics such as smoking cessation, weight management, and disease prevention.
- Quality Improvement:
- Participate in quality improvement initiatives and patient safety programs within the pharmacy.
- Monitor patient outcomes and provide feedback for improving services and patient care.
Qualifications:
- Education: High school diploma or equivalent required; a degree or certification in pharmacy technology, healthcare management, or related fields is a plus.
- Experience: Experience in a pharmacy, healthcare, or customer service environment is preferred.
- Knowledge: Familiarity with pharmacy practices, insurance processes, and medical terminology.
- Skills: Strong organizational, communication, and interpersonal skills; proficiency in using pharmacy management software or electronic health record systems.
- Certifications: Pharmacy Technician Certification (CPhT) or Medication Therapy Management certification is preferred.
