We’re looking for a Care Manager / Social Worker to join our team and help us expand our reach to serve more seniors in our community. In this role, you’ll act as a bridge between our agency and the healthcare professionals, social workers and community partners who help us connect with those in need. As a Care Manager / Social Worker, you will play a vital role in connecting seniors and their families with the care they need. You’ll build and nurture relationships with referral sources—such as healthcare professionals, discharge planners, and social workers—while advocating for the well-being of seniors in our community. Your work will directly contribute to enhancing the quality of life for older adults.

Key Responsibilities:
  • Community Engagement: Host and participate in events (e.g., support groups, appreciation events, CEU sessions) to educate referral sources and the community about our services.
  • Relationship Building: Conduct at least 30 face-to-face meetings per week with referral sources to foster trust and collaboration. Attend networking events to expand our community partnerships.
  • Advocacy & Support: Serve as a resource for referral partners by providing information, addressing concerns, and solving problems. Follow up proactively to ensure their needs are met.
  • Client Connections: Develop and maintain a network of long-term referral sources to help identify seniors who could benefit from our services.
  • Collaboration: Work closely with our care team to ensure seamless communication about client needs and updates. Maintain daily communication with office staff and management to foster a team environment.
  • Education & Growth: Stay informed about aging services, elder care trends, and community resources through professional development, reading, and networking.
  • CRM Management: Use our customer relationship management (CRM) system to track interactions, plan follow-ups, and evaluate referral source needs.

Who We’re Looking For:
We’re seeking a compassionate, driven individual who is energized by helping others and building connections. You should have a heart for serving seniors and a knack for fostering trust and collaboration.

Qualifications:
  • Experience in social work, care management, or a related field (healthcare, elder care, or senior services preferred).
  • Strong interpersonal and communication skills—able to build relationships with healthcare professionals, social workers, and community partners.
  • Knowledge of senior care, aging services, or home care is a plus.
  • Comfort with CRM systems and organizing outreach efforts.
  • Ability to work independently, prioritize tasks, and meet goals (e.g., 30 weekly meetings).
  • Passion for advocacy, problem-solving, and community engagement.
