Case Manager

Sign on Bonus: $600 for full-time (Half paid out at 3 months and other half at 12 months). 

Salary Range: $50,000-52,000 annually  

Position: 180 Degrees, a Human Services Non Profit, is seeking a Case Manager to work with youth in a 24/7 residential emergency shelter at St. Cloud

Responsibilities: 

• Ensures clients are compliant with court orders and remain law abiding.

• Meets with clients and families frequently once placed in shelter.  Monitors client progress as shelter resident, at school, and in the community.
• Identifies client and family needs and makes appropriate referrals to other county or community agencies.
• Creates comprehensive case plans and transition plans that reflect the needs of clients and families.
• Documents client contact and service coordination in an accurate and timely manner.
• Works effectively and collaboratively with other professionals in the field.
• Transports or arranges transportation for clients to medical, court, or other appointments.
• Maintains knowledge of culturally specific services available in the community.
• Maintains knowledge of the juvenile justice system, relevant laws, regulations and policies regarding social services provisions, reporting requirements, data practices, and client rights.
• Maintains flexibility to work evenings, holidays, and weekends as needed to meet the needs of clients and families.
• Demonstrates effective teamwork and collaboration with management and co-workers.

Location This role is in-person at our St. Cloud location.

Qualifications:  

• Minimum age: 21.
• Bachelor’s degree in psychology, social work, human services or related field preferred. 
• Two year working with youth preferred. 
• Dependable transportation for in-person work.
• Valid driver’s license and insurance. 
• Ability to clear a Department of Human Services background screening. 
• Lived experience in place of certain credentials will be considered for all positions.
