Position Overview

We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals facing various challenges, including behavioral health issues and addiction. This position requires strong communication skills, crisis intervention expertise, and a commitment to helping clients navigate their paths to recovery and stability.

Key Responsibilities

  • Conduct comprehensive assessments to identify client needs and develop individualized care plans.
  • Provide crisis intervention services to clients in urgent situations, ensuring their safety and well-being.
  • Facilitate access to behavioral health resources and addiction counseling services.
  • Collect and maintain accurate data on client progress, treatment plans, and outcomes.
  • File necessary documentation in compliance with organizational policies and regulatory requirements.
  • Collaborate with multidisciplinary teams to coordinate care and support for clients.
  • Communicate effectively with clients, families, and external agencies to advocate for client needs.
  • Utilize conflict management techniques to resolve issues that may arise during the case management process.
  • Stay informed about community resources and services available to clients.

Qualifications

Required:

  • Bachelor’s degree in Social Work, Psychology, or a related field is preferred; relevant experience may be considered.
  • Valid driver’s license and reliable transportation
  • Ability to pass a pre-employment drug test

Preferred:

  • Experience in crisis intervention and behavioral health is highly desirable.
  • Knowledge of addiction counseling practices is a plus.
  • First Aid certification is beneficial but not mandatory; willingness to obtain certification is required.

Skills

Technical Skills:

  • Proficiency in using Sunwave or willingness to learn
  • Proficiency in using Google Workspace

Soft Skills:

  • Strong data collection skills with attention to detail for accurate record-keeping.
  • Excellent communication skills, both verbal and written, to effectively interact with clients and stakeholders.
  • Ability to work collaboratively within a team environment while also being self-motivated.
  • Strong conflict management skills to handle challenging situations professionally.
  • Strong problem-solving and critical thinking abilities.
  • Excellent verbal and written communication skills.
  • Ability to work independently and as part of a team.
  • Strong attention to detail and organizational skills.
  • Ability to thrive in a fast-paced, deadline-driven environment.
