Position Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals facing various challenges, including behavioral health issues and addiction. This position requires strong communication skills, crisis intervention expertise, and a commitment to helping clients navigate their paths to recovery and stability.
Key Responsibilities
- Conduct comprehensive assessments to identify client needs and develop individualized care plans.
- Provide crisis intervention services to clients in urgent situations, ensuring their safety and well-being.
- Facilitate access to behavioral health resources and addiction counseling services.
- Collect and maintain accurate data on client progress, treatment plans, and outcomes.
- File necessary documentation in compliance with organizational policies and regulatory requirements.
- Collaborate with multidisciplinary teams to coordinate care and support for clients.
- Communicate effectively with clients, families, and external agencies to advocate for client needs.
- Utilize conflict management techniques to resolve issues that may arise during the case management process.
- Stay informed about community resources and services available to clients.
Qualifications
Required:
- Bachelor’s degree in Social Work, Psychology, or a related field is preferred; relevant experience may be considered.
- Valid driver’s license and reliable transportation
- Ability to pass a pre-employment drug test
Preferred:
- Experience in crisis intervention and behavioral health is highly desirable.
- Knowledge of addiction counseling practices is a plus.
- First Aid certification is beneficial but not mandatory; willingness to obtain certification is required.
