We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or RN to join our dynamic team. As an LPN or RN, you will play a vital role in providing high-quality patient care in a home healthcare setting. Schedule: Sunday 7am-7pm
Duties:
- Provide direct patient care, including administering medications, performing treatments, and conducting vital signs monitoring.
- Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans that cater to the unique needs of each patient.
- Manage and coordinate the flow of medical records and documentation, ensuring timely and accurate submission of patient information.
- Stay updated on the latest medical terminology and technology to provide the best possible care.
- Demonstrate a commitment to delivering exceptional care by actively listening to patients, addressing their concerns, and advocating for their needs.
Skills:
- Experience working in a home healthcare setting.
- Strong understanding of medical records and the importance of accurate documentation.
- Familiarity with infection control protocols and procedures.
- Strong comprehension of medical terminology and ability to use it effectively.
- Ability to work effectively with pediatric patients and their families.
-experience in trach/vent/Gtube is highly preferred

Qualifications
- LPN (Licensed Practical Nurse) certification
