Job Overview
1st Americare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our home health team. As an LPN, you will provide skilled nursing care to patients in their homes, ensuring they receive the highest quality of care while maintaining their independence. You will work under the supervision of a Registered Nurse (RN) and collaborate with other healthcare professionals to promote optimal patient outcomes.
Duties & Responsibilities
- Provide direct nursing care in accordance with the patient’s plan of care.
- Administer medications, treatments, and injections as prescribed.
- Monitor patient conditions, assess symptoms, and report changes to the RN or physician.
- Assist with wound care, dressing changes, and catheter care.
- Educate patients and families on disease management, medications, and home care techniques.
- Document all patient interactions and care provided in accordance with state and federal regulations.
- Collaborate with interdisciplinary teams to develop and adjust patient care plans.
- Ensure compliance with home health policies, procedures, and safety protocols.
- Travel to patient homes within assigned service areas.
Qualifications & Requirements
- Active LPN license in the state of Indiana (in good standing).
- Minimum of one (1) year of clinical nursing experience (home health experience preferred).
- Proficiency in wound care, medication administration, and patient assessments.
- Strong communication and critical-thinking skills.
- Ability to work independently and manage time effectively.
- Valid driver’s license and reliable transportation.
- Ability to pass a background check and drug screening.
