What are the responsibilities and job description for the LPN Home Health Nurse- Monday to Friday (8AM - 4 PM or 9 AM - 5 PM) position at 1st Ameristaff?
1st Americare is seeking an LPN for the weekday shift to work with our clients in the Round Hill location. Candidate with a G-tube/Trach/Vent experience is preferred.
The schedule for this case will be:
- Mondays to Fridays 8AM - 4 PM or 9 AM - 5 PM.
- Weekends as needed.
Duties and Responsibilities
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans accordingly
- Educate patients and their families on managing their conditions and promoting wellness
- Collaborate with other healthcare professionals to coordinate patient care
- Document patient care activities accurately and timely
Experience:
- Minimum of 2 years of experience as a Licensed Practical Nurse
- Experience with tube feeding and managing enteral nutrition
-Experience with G tube/Trach/Vent
- Knowledge of acute pain management techniques
- Familiarity with family planning services and counseling
- Knowledge of sterile processing techniques and infection control protocols
- Experience working with children in a healthcare setting

Pay: $35.00 - $45.00 per hour
