Job Summary
The Home Health Community Liaison plays a vital role in fostering relationships between our organization and the community. This position is responsible for promoting public health initiatives, educating the community on available resources, and facilitating program development. The ideal candidate will possess strong leadership skills and a passion for community engagement, particularly in the area of skilled home health care and rehabilitation services.
Duties
- Develop and implement outreach strategies to engage community members and promote programs.
- Educate individuals and groups about available resources, services, and programs related to in home rehabilitation.
- Collect and analyze data via Trella to assess impact and potential referral sources.
- Collaborate with local organizations, hospitals, rehabilitation centers, skilled nursing facilities, assisted/independent living facilities, physicians, etc.
- Oversee project management tasks to ensure timely execution of rehabilitative services.
- Provide administrative support as needed, including maintaining records and preparing reports.
Experience
- Proven experience in community outreach and business development within the hospice/home health or related markets (1yr preferred)
- Robust healthcare understanding preferred.
- Self motivated with a desire to win.
- Comfortable with utilizing and interpreting data technology to drive business development.
- Excellent communication skills, both verbal and written, with the ability to educate diverse audiences effectively.

Job Type: Full-time
Pay: $60,000.00 - $70,000.00 per year
