What are the responsibilities and job description for the Case Manager position at 1st Choice Facilitator Services?

We are seeking a Case Manager to monitor in-home services provided to consumers receiving Consumer-Directed Waiver Services to ensure the health, safety, and well-being of the consumer while the consumer remains independent in the community. Open positions in Bedford County, Campbell County, Halifax County, Charlotte County, Pittsylvania County, and city of Lynchburg. Position is part-time with a flexible schedule. The rate of pay is $21.25 per completed visit.
Responsibilities and Duties:
· Complete monthly in-home visits
· Document visits using designated DMAS documents and the company’s CSM database
· Identify needs and make necessary referrals
· Support the consumer in developing and monitoring a Plan of Care for personal and respite care
Qualifications:
· Minimum of Associates Degree in a health or human services field
· 1-3 years experience working with the elderly and/or disabled
· Ability to communicate with consumers, providers, and agencies
· Good organization and time management skills.
· Experience with Microsoft products
Requirements:
· Reliable transportation and a valid Driver’s License (must conduct face-to-face visits)
· Successfully pass 2 background checks with no barrier crimes
· Access to internet in home as work from home is required
